**39558BR** **Extended Job Title:** Sr Outreach Worker **Org Level 1:** Texas Tech Unv Hlth Sci Ctr El Paso **Position Description:** This position will serve as primary contacts for medically needy patients. Responsibilities include processing provider claims, using computer system to record and report patient information, patient case finding, financial screening, program enrollment facilitation, non-medical counseling, case tracking, and preparing all necessary case documentation. Persons in this position will serve as program spokespeople and representatives to participating health care providers, local communities and local health agencies; therefore, this position requires a positive and professional attitude when working with patients, community leaders, and local health agency and state agency care workers. Persons in this position will be expected to travel between local health-agencies in the fulfillment of their duties and may be asked to make occasional home visits. Work is performed in usual clinic conditions; requires a high degree of contact with patients and families, clinic staff and outside support agencies. Travel and ability to respond to needs during off- duty hours may be required in unusual situations. **Requisition ID:** 39558BR **Travel Required:** Up to 25% **Major/Essential Functions:** + Position will function as a outreach worker/navigator for under-insured and uninsured patients needing cancer screenings, diagnostic testing and care for any cancers diagnosed. + The navigator will be responsible for scheduling appointments, compiling notes, monitoring patient treatment plans, will use a computer application system to record and track patient information and attend medical appointments. + In addition, they will serve as a resource in advising patients about community resources for those with personal/environmental barriers to complete screening/diagnostic and treatment appointments. Implements cancer prevention education programs while adhering strictly to protocol in clinic, community and health fair settings. + Identifies participant personal and environmental barriers and assists participants and their families in identifying solutions to the barriers. + Becomes familiar with all training materials and resources that will be utilized in the implementation of the program. + Works in close collaboration with and communicates well with project team, other health care personnel and all collaborating partners in implementing the Program. + Seeks out new community sites and maintains excellent rapport and communication with all sites, adhering to their policies and procedures. + Completes all necessary paperwork, including consent forms and study related materials such as patient surveys. + Maintains adequate records of all participant encounters as per protocol. + Navigating patients through the health care system for cancer screening, diagnosis and treatment: arrange appointments, referrals, and follow-ups. + Track participants according to project protocol and documents appropriately. + Maintains adequate follow-up with participants. Identify and facilitate social service referrals such as transportation, child care, insurance. Identifies participant personal and environmental barriers and assists participants and their families in identifying solutions to the barriers. + Works in close collaboration with UMC, TTUHSC physicians, Project Director and other health care personnel in patient screening and treatment. + Assists in maintaining adequate follow-up with participants. + Works closely with program director or supervisor. + Functions under the immediate supervision of a Research Associate and Project Director with whom close communication is needed. + Have reliable source of transportation to conduct outreach activities and attend appointments with participants as required by the program; must have a current Driver's License and vehicle insurance. + Performs other tasks and duties assigned by the supervisor within the policies of the department and TTUHSC in order to meet the goals and objectives of the program and the department. + Adhere to all TTUHSCEP policies, procedures and processes Personally demonstrate, display, and act in accordance with TTUHSCEP values (Service, Respect, Accountability, Integrity, Advancement, and Teamwork) + Perform all other duties as assigned. **Grant Funded?:** Yes **Minimum Hire Rate:** Compensation is commensurate upon the qualifications of the individual **Pay Basis:** Hourly **Work Location:** El Paso **Preferred Qualifications:** + Interact effectively with a broad range of people at all levels: conduct community outreach activities in a variety of settings, including evening and weekends, and must be able to speak in public. + Possess excellent communication skills and interpersonal skills, and be a team player. + Attention to detail in record keeping, documentation, and reporting is vital to this position. + Have knowledge of the health care system and familiarity in navigating patients through the system for services; have extensive work experience and knowledge of people, organizations, and institutions in the El Paso, Texas and surrounding areas. + Ability to communicate in both English and Spanish is an essential part of this job to ensure effective communication with all program participants. + Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. + Ability to deal with problems involving several concrete variables in standardized situations. display a professional manner. + Have a high degree of expertise and ability to work with web-based applications, data management software and be able to complete data entry accurately. + Bilingual written and oral communication skills, experience and ability to work with different web-based applications, excel, spreadsheets, and office programs. **Campus:** HSC - El Paso **Department:** Family Medicine Elp Genl **Required Attachments:** Resume / CV, Transcript **Job Type:** Full Time **Pay Statement:** Compensation is commensurate upon the qualifications of the individual selected and budgetary guidelines of the hiring department, as well as the institutional pay plan.
