Job Overview:
Provides culturally informed services to in the outpatient service by evaluating via psychosocial assessments the social, emotional and interpersonal needs of the clients. Delivers direct clinical services such as Individual, Group, or Family Psychotherapy, couple counseling, crisis intervention, case management, outreach services and serves as a metal health consultant. Service delivery reflects all relevant age/developmental considerations.

Current NJ LSW or LCSW licenseEssential Functions:
Participates in all crisis intervention, crisis management action per the programs policies and procedures. Develops comprehensive history on the patient, including psychosocial development, current functioning, presenting problem, and history of treatment, through interviews with the patient, family and/or significant others. Develops written, comprehensive, psychosocial assessments based on the patient s history and relative to the presenting problem(s), reflecting age/developmental considerations. Responsible for development, implementation, monitoring, and documentation, in a timely fashion of initial interdisciplinary treatment plans. Consults with psychiatrist, program director, Program Coordinator, psychologist and other staff as needed for patient care. Delivers therapeutic services identified in the treatment plan which may include one or more of the following: individual, group, family psychotherapy, couple counseling, crisis intervention, screening, outreach. Responsible for providing case management services to clients when indicated in the treatment plan including: information, referral, linking, advocacy, planning, monitoring, and follow-up. Responsible for maintenance of assigned patient charts, ensuring all necessary documentation is present within established timeframes. Terminates with assigned patients as per the treatment plan, ensures appropriate termination, follow-up, and timely documentation. Presents closed MR s to supervisor on time and fully compliant. When indicated, provides crisis/scheduled outreach services in community or in the home of the patient and family. When indicated, participates in scheduled evening/weekend needs of the patients, at times in home or in community of the patient to meet the needs. Participates in crisis management response per program s protocols for clients dangerous to self/others/property. Attends weekly supervision. Takes active role in learning. Participates/presents in case conferences. Participates in departmental clinical, administrative meetings, and Grand Rounds. Takes responsibility for professional growth and development through ongoing participation in educational programs on-site and off-site, including clinically overseeing interns in the program. Maintains established departmental/hospital policies and procedures/objectives, quality assurance, safety, environmental and infection control standards. Meets assigned productivity requirements. Completes charge tickets as assigned, meeting program policy for timeliness. Provides data on services to supervision on time as required. Where assigned, uses computer to enter data, schedule appointments, and coordinate care. Meets customer satisfaction goals.Pay Range:
$ 38.42 Hourly Rate
