Responsibilities

In coordination with supporting the Vice President of Community Health Ministries (CHM), develop and execute the vision for providing key direction to health equity-driven performance strategy and population health interventions within Ascension’s CHM portfolio. As part of the Leadership team, partners with Clinical Leadership, Operations and Program leads across multi-state clinical sites to develop and implement value-based programs, monitor performance metrics and accountability, and improve performance measures as needed. Responsible for supporting the VP in leading program implementation for value-based care, population health, specialty value-based care solutions and CHM reimbursement innovation. 

 

Skills:

Healthcare Analytics expertise - Hands on experience with analyzing healthcare data, statistical and observational studies, Value-based care subject matter expertise - Direct experience and knowledge of CMS value-based models in primary care and specialty care supporting program designs (ACO, Medicare-Shared Savings, etc.)Healthcare subject matter expertise - Direct industry experience through health care delivery networks, managed care administration, community health, health IT management; strong healthcare and / or health plan business acumen for developing actionable guidanceLeadership and Organization - Experience in managing 10+ teams of resources. Exceptional capacity for managing and leading people and cross-functional data science and business analytics teams; capacity to enforce accountability, develop and empower top-notch analytics staff from the bottom up, lead from the top down, cultivate entrepreneurship, and learn the strengths and weaknesses of the team so as to put people in a position to succeedCommunication - Strong persuasion skills and effective communicator; ability to create and communicate a compelling vision internally and externally



Responsibilities: 

Accountable for the program performance, administration, and operations of established value-based care CHM programs. Work closely with clinical and operational teams to integrate data-driven approaches into care delivery and management practices. Provides leadership for improving access to care for uninsured and vulnerable populations consistent with mission, principles, and call to action. Provides actionable improvement opportunities through ongoing opportunity analytics and helps drive the actions from the providers to improve performance.Foster genuine teamwork and a sense of common purpose among all staff, through appropriate delegation, decision-making, clarity in roles, and team collaboration which ensures that tasks are completed efficiently and with respect for all- aligning with our mission, principles and values.Responsible for creating, managing and deploying the financial management reporting to drive visibility on program financials success, opportunities and evaluate the effectiveness of value-based care initiatives and recommend strategies for optimization. Oversee and collaborate with the data analytics team to the collection, analysis, and interpretation of healthcare data related to value-based care programs.Develops, oversees, monitors, and evaluates major community health ministry improvement programs and initiatives. Develop metrics and methodologies to measure the impact of interventions and identify areas for improvement. Communicate findings and recommendations effectively to Ascension stakeholders, including senior management and vendor partners. Provide leadership of data governance issues and resolution processes to ensure data accuracy and timeliness. Institute standardization and quality assurance across all aspects of delivery, including financial modeling, evaluations, and opportunity analysis.Offer authoritative position on strategic planning for developing value-based care and community health solutions across the organization.Ability to prioritize multiple project types and work streams, all for purposes of decision making and execution across different parts of the organizationExecute the talent strategy required to achieve business needs and organizational goals.Reviews industry trends and recommends cost effective and efficient solutions utilizing knowledge of CMS value-based models in primary care and specialty care supporting program designs (ACO, Medicare-Shared Savings, etc.)
Requirements

Education:

High School diploma equivalency with 5 years of applicable cumulative job specific experience required, with 2 of those years being in leadership/management OR Associate's degree/Bachelor's degree with 3 years of applicable cumulative job specific experience required, with 2 of those years being in leadership/management.
Additional Preferences

Education/Experience:

10+ years of hands-on experience in managing staff in healthcare working with clinical, administrative and information technology personnel. data (e.g., payer claims data or billing data).Bachelor’s degree preferred with 10+ years of experience in public health, community health, healthcare administration or business administration.Master’s degree strongly preferred in business/health administration, public health, or economics preferred.

Certification/Licensure:

Certified PMP, preferred.
