The Sr YV LifeSet Specialist role is to support YAs become self-sufficient by assisting in locating and learning how to use community resources such as housing, employment and education. The Sr LifeSet Specialist will also assist the supervisor in leading a team of specialists utilizing the LifeSet model.


$58,905-$66,205 

Responsibilities

1. Engage YA w/ a background in Child Welfare by building & maintaining trusting relationship with a caseload of 8-10 YAs 

2. Conduct weekly sessions to provide support & service planning with YAs as well as conduct monthly assessments by developing monthly service plans to address treatment planning with YAs

3. The ability to establish and maintain collaborative working relationships with colleagues, & stakeholders in the community 

4. Attend weekly coaching, consultation & group supervision with LPE/SUP to receive appropriate feedback on next steps wih planning for each YA in the program 

5. Attend agency/ACS/LifeSet Booster training ongoing to support with professional development 

6. Complete timely and accurate documentation of YAs progress as it relate to TX Plans, Why What Asssesments, PEAs, Screeners, & Session Notes in CONNXs, & Evolv database systems

7. Ability to support shadowing opportunites & some coverage support to new & exisiting specialist as directed by supervisor 

8. Support supervisor in administrative tasks as needed, as well as contribute to the overall leadership/cohesiveness of the team to ensure the the program's model fidelity is met
