Clinical Staff Pharmacist provides clinical, order entry and distributive functions in a decentralized clinical staffing model. Provides clinical consults, kinetic dosing, and recommendations/ interventions to providers for assigned patient care units. (Med Surg units, ED, Critical Care, Orthopedics, Womens Care, Cardiovascular units, Behavioral Health, Outpatient Infusion, Pain clinic and Surgical patients. Responds to medical emergency codes. Supervises technician team for distributive operations in Central Pharmacy, including sterile compounding service. Verifies CPOE orders. Performs med rec at admission/discharge to facilitate safe transition. Counsels patients. Provides drug information to providers. Participates in pharmacy shared governance team to promote performance improvement. Optional precepting of APPE students or PGY1 residents.
EQUIPMENT USED/SPECIAL SKILLS REQUIRED:
Hospital Pharmacy experience with IV admixture, code response, kinetic dosing, automated dispensing cabinets and computer experience highly recommended.
MINIMUM QUALIFICATIONS OR EQUIVALENTS:
Education: Must have graduated from an accredited Pharmacy school in the US. Pharm.D. preferred.
Experience:
Must have sufficient experience in all areas of hospital Pharmacy to successfully fulfill the responsibilities of the position. PGY1 Residency training or three (3) years experience as a Clinical Pharmacist in an acute care hospital setting is preferred. Experience in precepting Pharmacy Students and Residents is beneficial.
LICENSES, CERTIFICATIONS, AND/OR REGISTRATIONS:
Kentucky Board of Pharmacy License. Certified provider of Basic Life Support (BLS) within 90 days of employment. ACLS within one (1) year of employment.
