Position Summary:

Functions in a clinical lead role in collaborative interdisciplinary team providing comprehensive care to patients and families with complex healthcare needs that may be both predictable and unpredictable.  Aims to provide care that is safe, effective, patient-centered, timely, efficient, and equitable.

Position Key Accountabilities:

1.  Performs responsibilities in a manner that promotes quality patient care and customer service/satisfaction.
2.  Adheres to broad scope of practice as established by the Nurse Practice Act.
3.  Assures compliance with quality standards, and regulatory and accrediting agency specifications to meet federal, state and local standards.
4.  Assists clinical  and administrative staff in formulating goals and objectives .to meet both qualitative and quantitative outcomes.
5.  Participates in the development of nursing and clinical policies procedures, and competencies. Develops and/or acquires instructional materials, supplies, and equipment necessary to conduct training continuing education and staff development. 
6.  Prepares and maintains records and reports concerning operations.  Remains abreast of developments and evidence based practice  by attending seminars and workshops, communicating with peers, and reading pertinent literature.  Incorporates such advancements in continuing education and nursing practice.
7.  Ensures the efficient operation of the clinic or unit by proper scheduling and planning for optimum use of equipment, staff and resources, maintenance of stock and supplies, efficient and safe use of equipment.
8.  Uses systematic approach to provide nursing care to patients and families/populations by performing focused and comprehensive assessments through subjective and evidence based objective data.
9.  Prioritizes, synthesizes, and documents assessments and nursing plan of care using critical thinking and judgment.
10.  Implements plan of care within scope of practice through assignment and monitoring of other nurses and delegation and supervision of unlicensed staff.
11.  Initiates standing orders for prescription refills and labs as well as standing delegation orders where appropriate.
12.  Develops and implements teaching plans concerning maintenance, promotion, and restoration of health.  Evaluates patients’ responses and outcomes to education and therapeutic interventions.
13.  Communicates effectively with collaborative team while escalating critical results and issues in an appropriate and timely manner.
14.  Exercises judgment while coordinating care and referring patients to agencies or health care professionals for other services/transitions in care where indicated.
15.  Performs onsite and telephone triage using best practice guidelines and documentation to ensure patients receive the right level of care in the right setting at the right time by the right caregiver using the right equipment and supplies.
16.  Acts as a patient and healthcare advocate by maintaining a safe environment and promoting quality healthcare and services.
17.  Respects and maintains privacy and confidentiality per HIPAA guidelines.
18.  Adheres to the standard of practice for professional nursing.
19.  Acts as a clinical and leadership resource for team members.
20.  All other duties as assigned.

Certification/Skills:

Current nursing licensure in the State of Texas.
Basic Life Support (BLS) Certified
Advanced Cardiac Life Support (ACLS) Certified preferred

Minimum Education:

Bachelor’s degree in nursing from an accredited School of Nursing plus two year of nursing experience, OR,
Associates degree in nursing from an accredited School of Nursing plus four years of nursing experience, OR, Nursing diploma (three year program) plus three years of nursing experience.

Minimum Experience:

Two (2) years of nursing experience with a BSN.
Three (3) years of nursing experience with a Nursing Diploma.
Four (4) years of nursing experience with an ADN.

Physical Requirements:

Exerts up to 50 pounds of force occasionally and/or up to 20 pounds frequently and/or up to 10 pounds constantly to move objects.
