Description:

POSITION SUMMARY:
Contributes to the care of the patient by developing pharmacotherapeutic regimens and counseling utilizing pharmacology, pathophysiology, pharmacokinetics, and interpersonal communication skills. Provide clinical pharmacy and drug information services to serve hospital staff and collaborate with physicians. Engages in quality improvement and professional growth to meet hospital, departmental, and personal goals. Compound, package, and dispense medications and IV solutions and preparations as ordered. Assist in the supervision of pharmacy technicians and interns in the operations of the pharmacy. Acts as a resource to students of various health disciplines.

JOB SPECIFIC ESSENTIAL FUNCTIONS

1. Works towards achievement of department-wide pillar goals.
2. Assist in the training of pharmacy residents and new pharmacists.
3. Supervise Pharmacy Interns and Technicians to ensure proper training and job performance according to established policy, procedure, and protocols.
4. Work collaboratively with physicians, nurses, and other staff in both central pharmacy and unit-based practice to ensure effective pharmacotherapy.
5. Ensure patients are knowledgeable about new medications and the side effects of all medications that they are taking.
6. Perform provider and patient education in a manner that is service-oriented and consistent with MGHP values.
7. Participate in community outreach as a member of MGHP and the Department of Pharmacy; Appropriately documents community service
8. Carry out specific clinical protocols, such as IV-to-PO conversions and therapeutic substitutions, as defined by Pharmacy and Therapeutics.
9. Demonstrates through leadership and support a “servant” attitude that exemplifies the philosophy, mission, values and vision of MGHP and Trinity-Health
10. Compound and dispense prescription orders from providers, ensuring patient safety, accuracy, and preventing drug incompatibilities for oral and parenteral medications.
11. Interact professionally and effectively with team members to ensure a collaborative plan of services. Keep supervisors and employees informed about issues.
12. Maintain required records and automated dispensing cabinet inventories for controlled substances. Helps investigate controlled substance discrepancies.
13. Designs pharmacokinetic dosing regimens and pharmaceutical care plans. Appropriately documents plans and interventions.
14. Obtain and maintain competency in accessing and documenting patient information in the automated patient medical record as defined by departmental policies.

15. Explores ways in which to expand clinical pharmacy services and relationships and foster growth within the department, MGHP, and the community.
16. Demonstrate self-directed learning and parti-cipation in continuing education/research to ensure professional growth.
17. Actively promote and participate in various task forces, teams, or committees in the department or for the hospital at large. Participate in various events representing the organization to the community.
Financial
18. Steward hospital resources effectively in terms of process improvements and medication costs; aids in Pharmacy and Therapeutics initiatives.
19. Serves as process Resource for Revenue Center about charging and assumes daily responsibility to complete.
20. Serves as process Resource for Revenue Center about error correction and assumes daily responsibility to complete.
21. Serves as process Resource for Resource for Revenue Center about revenue reconciliation and assumes daily responsibility to complete.
22. Demonstrates proficiency in utilizing System generated and/or Business Object Reports to validate and substantiate revenue center revenue performance.
23. Interacts with CMD coordinator for alignment of services provided being consistent with services charged; participates in the identification of new charge codes
24. Assures that processes are aligned whereby entered charges are consistent with actual services provided to patients.
POSITION QUALIFICATIONS
1. BS degree in Pharmacy. Licensed to practice in the State of Michigan. Advanced degrees (i.e., Doctor of Pharmacy) and residency training preferred.
2. Demonstrated ability to interact appropriately and effectively with patients, family members, healthcare team members, ancillary departmental staff, medical staff, visitors and the public at large.
3. Demonstrated ability to perform duties in a safe manner, adhering to policies and procedures in the delivery of patient care.
4. Able to identify patient care concerns and problems outside scope of responsibility and initiate appropriate corrective action.
5. Ability to change work priorities in consultation with supervisor as the situation warrants. Able to adapt to changing patient and department needs. Work environment requires considerable flexibility.
6. Articulate and demonstrate commitment to the philosophy, values, mission and vision of MGHP.
7. Demonstrated commitment to hospital pharmacy practice, as demonstrated by recent (within 5 years) employment in hospital practice or advanced clinical pharmacy work.
