The **HARBORVIEW MEDICAL CENTER PHARMACY DEPARTMENT** is looking for an outstanding **STAFF PHARMACIST - INPATIENT/TRANSITIONS OF CARE** .
**WORK SCHEDULE**
• Full-Time/40 hours per week
• Rotating/variable shift
**POSITION HIGHLIGHTS**
• Be an integral member of the Pharmacy team and provide transition of care services to multidisciplinary teams, optimizing safe and fiscally responsible transitions of care
• Responsibilities include, but are not limited to, drug distribution and transitions of care facilitation aspects of the care of Harborview patients
Strong candidates will have completed an ASHP-accredited residency program or advanced degree.
**PRIMARY JOB RESPONSIBILITIES**
• Participate as an active member of the pharmacy team to assist in successful transitions of care for complex patients at high-risk of readmission
• Identify and resolve medication discrepancies to ensure an accurate medication list at discharge for select patients
• Facilitate communication with providers upon patient transition of care into the outpatient setting
• Assist patients in navigating insurance coverage to minimize barriers to accessing prescribed therapies
• Refer high-cost medications to outpatient billing team to manage prior authorization (PA) requests for medications, ensuring timely approval and reducing treatment delays
• Participate in medication order review, clarification, validation, and order verification
• Supervise inpatient pharmacy technicians in all areas
**REQUIRED POSITION QUALIFICATIONS**
One year of experience in pharmacy practice or completion of an American Society of Health Systems Pharmacists accredited residency training program.
**LEGAL REQUIREMENT**
State of Washington Pharmacist license.
