What you will Do:
Reviews electronic/written medication orders to determine clarity, appropriate dose, schedule, possible drug interactions and contraindications. Consults directly with the physician, physician/hospice nurse or nursing home, if changes or clarification is needed.Supervises preparation of patient medications using standard techniques and procedures, including all drugs, prescriptions, controlled substances, IV's and IV admixtures.Successfully interacts with interdisciplinary teams, administrators, physicians, residents, resident families, nursing directors, nursing staff, as well as state and federal regulations.Directs and monitors pharmacy technicians in proper packaging, labeling and distribution of medications.Successfully operates the outpatient computer system and prepares, fills and dispenses outpatient charity prescriptions as needed.Participates in drug therapy monitoring including evaluation of at least the following: a) appropriateness of route and method of administration, b) potential therapeutic duplication, c) drug-drug interactions, d) drug-food interactions, e) drug-lab interactions, f) drug-disease interactions, g) pharmacokinetics analysis of lab data, h) safety and efficacy of drug therapy, I) cost-effective therapeutic alternatives, j) therapeutic appropriateness of medications and k) assessment of patient diagnosis, medical history and any pertinent physical findings. Complies with OBRA standards for patient counseling on prescriptions.Maintains departmental medication inventory levels by checking stock and orders supplies and drugs as indicated. Maintains perpetual inventory system. Maintains invoices in orderly fashion to comply with legal standards. Checks pharmacy stock for outdates and proper amount of stock.Consults with and advises physicians and nursing staff on matters pertaining to the selection, procurement, distribution, storing and safe use of drugs.Maintains records as required by department, state and federal laws. Researches drug information for physicians, nursing, patients and other health care professionals.Minimum Qualifications:
Requires a Bachelor’s Degree in Pharmacy and licensure in the State of Iowa. Must have the ability to work independently and to set priorities, make critical decisions, and respond quickly to emergent situations.Must have basic typing skills and be familiar with computers and their functions.
