Statewide Program Manager 1: (Nursing Facility Clinical Liaison) This position is in the Louisiana Department of Health, Office of Aging and Adult Services (OAAS), within the OAAS Executive Management Team. The OAAS was formed within the Department of Health as a healthcare reform initiative, bringing together all of the long-term care programs that serve senior citizens and people with adult-onset disabilities. OAAS aims to develop, provide and enhance services that offer meaningful choices for people in need of long-term care. The office is committed to developing a long-term care system that provides choice, ensures quality, meets the needs of consumers and caregivers and does so in a fiscally responsible manner. OAAS has an annual administrative budget of over $60 million. It provides services to over 30,000 recipients through several large and complex Medicaid programs whose budgets total over $1 billion; including the Nursing Facility Admissions Program, Long-Term Personal Care Services Program, and two Medicaid HCBS waiver programs. OAAS also manages one state-operated health care facility, Protective Services, Program of All Inclusive Care for the Elderly (PACE) and other non-Medicaid grants and programs. The Statewide Program Manager 1 (Nursing Facility Clinical Liaison) manages complex nursing facility quality improvements involving multiple components statewide. The incumbent is highly responsible for building an infrastructure within OAAS to provide a state liaison for non-regulatory actions, as well as serving as the liaison to nursing facilities and nursing facility associations statewide. The incumbent is highly responsible for the management of technical assistance offered to nursing facilities, to manage quality improvement initiatives and projects within nursing facilities statewide and to assist with on-going projects initiated Incumbent must demonstrate working knowledge of nursing facility regulations and standards to provide technical assistance to facilities implementing quality assurance and performance improvement programs. Establishes statewide quality improvement goals and develops organizational plans to facilitate implementation. The incumbent is responsible for the statewide quality monitoring of nursing facility level of care determinations made by OAAS, ensuring consistent nursing facility level of care determinations are made statewide. Provides recommendations suggesting quality enhancement and training needs, specific to level of care determinations to OAAS Executive Management and OAAS Program Operations/Training Team. Examples of Work 40%Nursing Facility Liaison + Serves as statewide clinical liaison for nursing facilities and nursing facility associations, as well as statewide task forces, work groups, councils, and coalitions that address services for aging and disabled adults. + Travels to nursing facilities to develop relationships with facility staff and understand their quality improvement needs. + Develops current and long-range plans, policies and procedures for statewide nursing facility improvement and remediation through partnerships for improved quality. + Provides on-going consultation and technical assistance to professional and support staff on programmatic issues. + Establishes program goals and develops organizational plans with limited input for higher levels of management. + Responds proactively to internal audit findings, programmatic challenges and issues; coordinates responses and reports to auditors and federal/state governmental entities as requested. Meets with LDH internal teams; Office of the Secretary, Health Standards Section and LDH Legal to proactively understand internal audit findings, programmatic challenges and issues. + Facilitates communication between LDH and nursing facilities, to include preparation and writing of memorandums, policy and procedure manuals. + Provides technical assistance to inform revisions to nursing facility rules, regulations and procedures to meet changes in law or policy. + Produce and disseminate communication materials to nursing facilities statewide. + Revises rules, regulations and procedures to meet changes in law or policy. + Manages direct supervision over Clinical Program Manager 1A. 30%Manages Quality Monitoring + Develops monitoring tool to assess statewide nursing facility level of care determinations completed by OAAS. + Maintains nursing facility level of care certification issued by OAAS. + Completes quality monitoring of a representative sample of OAAS level of care determinations completed statewide to include desk reviews and face to face assessment types. + Provides recommendations to enhance statewide level of care determinations and inform Executive Management of strengths identified within determination practices. 20% + Assist in the implementation, ongoing work and monitoring of quality assurance standards as it relates to OAAS initiatives and nursing home admissions requests. + Provide clinical expertise to support ongoing efforts to develop and implement quality assurance procedures, monitoring performance and identifying/addressing quality issues of OAAS initiatives. + Identify training needs, develop procedures, and complete reporting of identified metrics. + Analysis of reports received from management tools designed to streamline and optimize statewide LOC determinations. + Provide on-going consultation and technical assistance to professional and support staff on programmatic issues. + Serve as liaison to ensure compliance with nursing home regulations and other departments within LDH.
