Responsibilities Step-Down Navigator Full-time Via Linda Behavioral Hospital, newly opened, is excited to begin offering a full continuum of inpatient and outpatient services to the Scottsdale, Arizona population and the greater Phoenix region. Our 120-bed facility offers specialized mental health services and substance use treatment for teens, adults and older adults experiencing issues such as depression, anxiety, personality disorders and co-occurring addictions. Student Loan Forgiveness through AZ BHLRP Position Summary: The Step-Down Navigator is responsible for meeting with all eligible patients to discuss the benefits of Outpatient services as part of their continuum of care treatment. The Step-Down Navigator will be responsible for communicating eligibility to key members of senior leadership & associated departments and will be responsible for routinely collecting and entering data, specific to Outpatient eligibility, into Midas for trending report purposes. They will also complete appropriate documentation in MS4 for inquiry calls for Outpatient. They will also assist with outcome reporting as needed. He/She will have a thorough understanding of the Step-Down Playbook, implementing components of the Playbook as needed. A thorough understanding of the Inpatient work-flow process, as it pertains to case management and discharge planning, is preferred. This position reports to the Director of Social Services. Qualifications Requirements Bachelor's Degree from an accredited university required. Master's degree preferred. A minimum of Two (2) years' experience in a psychiatric/mental health field, preferred.
