Job Summary :The Stroke Coordinator in collaboration with the attending physicians provides expert and complex nursing care to patients and their families, including evaluations, monitoring of treatment plans and patient and family education. The Stroke Coordinator is responsible for the quality of patient care, personnel management, management of assigned research projects and program goals.
Position Responsibilities
Works under direction of Clinical Outcomes Svc Line to facilitate program, project and specific education related to stroke. Real time follow up (when possible) with staff/units when there were delays in identification or opportunity for improvement related to compliance with interventions of the stroke guidelines.
Assist the rapid response team and other hospital staff, when necessary, if dealing with a patient situation in which there are obstacles preventing the appropriate stroke treatment from being provided.
Review data to identify trends and issues, also use improvement tools to assist with problem solving and action planning. Present data to Quality.
Collaborates/communicates with peer and interdisciplinary groups to develop practice standards and optimal patient care services. Identifies areas of improvement, develops implementation plan and educates clinical staff as appropriate.
Makes patient rounds with the medical staff on in-patients. Facilitates the implementation and monitoring of stroke protocols and all acute care. Determines baseline compliance with required physician documentation.
Provides patient, family and community education. Participates in community and health professional meetings and conferences to promote and enhance public awareness of Stroke, current therapies, as well as services and resources for patients and their families.
Implements a self development plan to maintain and upgrade advanced knowledge, skills and experience for Stroke patients and their families.
Position Qualifications Required:
Required Experience: Minimum of 2-4 years of clinical nursing experience, Critical Care experience preferred, Demonstrated experience with Shared Governance, Nursing Standards, Nursing Quality Outcome Practice Performance Improvement.
Required Education: Bachelor’s Degree required, Master’s Degree preferred
Training / Certification / Licensure: Registered Nurse or Nurse Practitioner with current NJ license, CPR and Membership in professional organization required, ACLS preferred
