Responsibilities

The cardiovascular nurse navigator is responsible for guiding select cardiovascular high risk patients from their pre through their post hospital care experience. The navigator functions directly under the CVS director. The navigator is responsible for organizing the relevant education of the patient and their family and serves as the cardiac surgery resource on behalf of cardiovascular services. The navigator assists with new program development and is responsible for data collection and extraction as directed by the CVS director.

Qualifications
Minimum:

• BSN

• 3 - 5 years of full time experience as a RN with critical thinking and working with high-acuity patient populations

• Current and valid California RN license • Current BLS certification • Current ACLS certification

• Experience in providing formal and informal physician and nurse education • Computer skills utilized in the health care environment

Preferred:

• Nurse Practitioner

• Experience in community care and chronic disease management
