Position Responsibilities: 

Responsible for the overall stroke program to follow all stroke care delivered. Ensures that all protocols and quality of care meet evidenced based guidelines. Keeps abreast of changes in regulations and required reporting, documentation and protocols. Reviews daily admission log for patients admitted with key diagnoses. Reviews charts to determine eligibility for stroke program. Reviews medical records concurrently and retrospectively and reviews data from Outcomes Sciences website accurately. Completes concurrent medical record reviews timely to ensure patient care meets all criteria of the program. Works with clinical staff to ensure patient care delivered meets current standards of care.  Advises immediate supervisor of any issues with physicians or staff refusing to follow established medical guidelines. Works with Manager, Medical Staff Quality on peer review of cases not meeting criteria. Works with clinical staff in ensuring standards are met with regard to stroke center accreditation. Facilitates and evaluates the quality of nursing care as it relates to stroke patients. Provides education and guidance to clinical staff on stroke signs and symptoms, need to follow treatment protocols, NIH stroke scale assessment tools and other guidance as required. Provides feedback to and collaborates with immediate supervisor on methods to improve compliance with stroke program requirements. Works with physicians, nurses and case managers on identifying patients with key diagnoses and ensures pre-printed physician orders are on the record and appropriate discharge forms have been implemented. Participates in monthly conference calls with the American Heart Association and American Stroke Association, other meetings as assigned Maintain awareness of regulatory requirements and ensure organizational compliance. Demonstrates knowledge of Stroke Program Accreditation in daily work.  Demonstrates understanding of principles of quality management and performance improvement in daily work. Utilizes these principals to develop and maintain quality and performance improvement relative to regulations and metrics Reviews Stroke Program Center accreditation standards to ensure facility compliance on an on-going basis. Prepares the center for survey activities, and writes responses with guidance of Director of accreditation Responsible for the coordination of performance improvement efforts with each nursing unit/educator serving as resource to nursing in care of the stroke patient. Serves as coordinator/educator of community outreach for stroke patients' needs in our community. Conducts twice a year community education for the Stroke Center. Facilitates the Stroke Services Committee and other committees as assigned. Assists managers in preparation for committees by preparing accreditation reports, policies, procedures and other documents as required. Ensures Patient Satisfaction system is in place for the Stroke Center population, reviews the results of the surveys, provides summary reports for committee review and action and follows up with patients as needed and required. Responsible for reviewing stroke performance measures, makes recommendations for improvement and ensures the data and applicable analyses and action plans are submitted to the Stroke Services Committee and PI department. Prepares data to present at MSQP as required Works with appropriate physicians in updating clinical practice guidelines that meet evidence based medicine protocols as established by the American Stroke Association Responds to Stroke Alerts, participates in assessing patients and providing appropriate direction in the clinical care necessary to treat the patients.  Educates the Rapid Response Team in Stroke Alert protocols. Works with neurologists, ED doctors, neurosurgeons and other physicians in the treatment of stroke patients.

Qualifications:

RN  BS in Nursing preferred At least 2 years in a similar role
