The role you’ll contribute:

Clinical Coordinator ensures that all necessary resources are in place to facilitate effective client support and treatments toward providing quality patient care. This means they interact daily with patients, physicians and nursing staff. The CC is responsible for all program management, including staff and community education if applicable, patient rounds, outcomes monitoring, preparation of statistical reports, data abstraction and maintenance of all required documentation; and compliance with national standards, certifications, and accreditations.

The value you’ll bring to the team:

· Coordinates care of patients in the program to ensure quality clinical outcomes achieving shorter length of stay, low morbidity and mortality rates as well as patient satisfaction and post-dc calls as applicable.

· Coordinates and provides physician, staff, patient and community education as applicable; evaluates the effectiveness of these programs when possible.

· Consistently uses current evidence-based guidelines, standing orders, and protocols to facilitate positive patient outcomes.

· Collaborates with other in-house departments and community resources to provide continuity of care, as indicated.

· Develops, monitors, analyzes and reports program performance improvement indicators and initiates process improvement as needed. Maintains ongoing QI plan.

· Acts as an internal and external consultant on program and patient population related initiatives.

Qualifications

The expertise and experiences you’ll need to succeed:

· Bachelor's degree required

· Minimum of three (3) years clinical experience in area of specialty

· Current Florida RN License

· AHA BLS- certified
