Position Summary:
The Substance Abuse Case Manager is a member of the Dignity Health care team. The care team program improves the quality of care and clinical outcomes for members with complex care needs by coordinating care within the health care delivery system, while collaborating with a multi-disciplinary team.
Through assessments to determine unmet needs and development of the individual's care plan, the Case Manager evaluates and identifies social service needs and is able to determine appropriate resources or services required to meet an individual's health needs, provides education/coaching on disease self management for health promotion and maintenance, monitors patient's progress, promotes quality cost effective outcomes with the goal of improved care coordination amongst providers and increased involvement of the individual/family/caregiver in the decision making process to reduce hospitalizations, readmissions and ER visits.
This position will involve telephonic case management and direct patient contact. Provides flexible, case management services to adults within Woodland Clinic Medical Group (WCMG) and Mercy Medical Group (MMG) within continuity of care context, including advocacy for services within the community and social services system.
Qualifications
Minimum Qualifications:
- A minimum of 2 years post licensure/certification, with an emphasis on mental health and substance abuse treatment.
- Certified Alcohol & Drug Counsler (CADC)
- Certified Addiction Specialist (CAS) OR Certified Alcoholism and other Drug Addictions Recovery Specialist
- Clinical training in an acute care or clinic environment.
- Demonstrated expertise in medical terminology, disease states
- Demonstrated problem solving, collaboration, management skills including coaching and through addiction treatment background. Development/training skills.
- Highly skilled in use of desktop software, including Excel and Word. Word processing, keyboarding and non scripted documentation skills. Able to operate PC-based software program.
- Demonstrated ability to independently assess, evaluate and interpret clinical information and care planning.
- Ability to perform job functions independently and with minimal supervision.
- Effective oral, written and listening communication skills necessary for constant interaction with patients, families, physicians and other members of the health team.
- Able to stay organized and flexible in a work environment with multiple assignments and changing priorities
- Must have reliable transportation to travel to various sites and patient appointments as needed.
Preferred Qualifications:
- 3 -5 years of broad clinical experience preferred. Experience working with all ages regarding substance abuse preferred.
- LVN or LPT preferred.
- Chronic disease management training preferred. Clinical training in managed care capitated healthcare environment preferred. Knowledge of evidence based clinical practice guidelines preferred.
- Preferred Special Skills: Possess high level people management and process management skill. Excellent management and negotiation skills. Knowledge of IDX/Flowcast and ALLSCRIPTS desirable. Experience with Chronic Medical Illness Medical terminology. Strong clinical assessment and critical thinking skills. Ability to demonstrate leadership skills to delegate and provide direction/guidance to multidisciplinary team. Knowledge of regulatory and accreditation standards and complex case management. Knowledge of capitation/HMO, insurance payors and government healthcare plans. Knowledge of community resources - Experienced with data collection, data entry, report generation and other computer systems and applications - Familiarity with an electronic practice management system/EMR.
