Substance Use Navigator

Know Your Rights: https://www.eeoc.gov/sites/default/files/2022-10/EEOC\_KnowYourRights\_screen\_reader\_10\_20.pdf Responsibilities The substance use navigator (SUN) helps patients access evidence-based treatment for substance use disorders with a focus on low-threshold access to MAT (medication for addiction treatment) and harm-reduction. SUNs conduct outreach and identification of patients, conduct initial brief assessments, introduce patients to MAT programs and services, facilitate appointments at outpatient MAT clinics, serve as a resource and coach for their clients, and advocate for culture change in the hospital. They also assist with access to other services such as financial counseling, primary care, mental health services, social services, and residential treatment facilities. The substance use navigator will work in a collaboratively style and approach with the Social Services I, Social Services II, Social Services III and the Case Manager in the Behavioral Health Department to meet the needs of our patients. The substance use navigator will also work with staff and/or providers in the Emergency Department, Crisis Stabilization Unit and other hospital or Shasta Regional Medical Group Providers. Assists with Discharge Planning as assigned by interviewing patients/families and gathering data to develop a safe patient-focused Discharge Plan. Advocate for a harm-reduction approach to patients who use drugs within the hospital and community to reinforce evidence-based, non-judgmental approaches so that patients who use drugs get the same care as patients who do not use drugs. Qualifications EDUCATION, EXPERIENCE, TRAINING ● Nonjudgmental, energetic, positive, harm-reduction approach to assisting patients with Substance Use Disorder (SUD) ● Understanding of SUD as a medical condition and the role of MAT in treatment and harm reduction. Adherence to an abstinence-based social model that is unsupportive of MAT is not aligned with the goals of this position. ● Ability to use computer information systems, computer literacy, and knowledge of word processing ● Interest/proficiency in working with individuals recently released from incarceration, homeless individuals, and individuals in other underserved populations. Prior experience working with individuals in this client group highly desired. ● Ability to interact with patients in a culturally relatable manner. Any additional ability to communicate in foreign languages spoken in the local community such as Spanish, Tagalog, Hmong, etc. would be helpful. ● Connections to community partners and various community resource groups in the local community highly desired. ● Respect for patient confidentiality and patient privacy is highly desired. ● Ability to communicate clearly verbally and in writing. ● Previous hospital experience as an admissions representative preferred ● Knowledge of medical terminology preferred ● Effective written and verbal communication skills is essential ● Ability to multi-task, prioritize needs to meet required timelines is critical ● Analytical and problem-solving skills is critical ● Ability to assemble relevant data, analyze that data and provide recommendations to the Behavioral Health Director ● Previous Customer Services experience required ● High School Graduate or GED Equivalent Required ● Prior college courses completed in Sociology, Psychology, Social Work or other related courses highly desired [Facility Name] offers competitive compensation and a comprehensive benefits package that provides employees the flexibility to tailor benefits according to their individual needs.
