Maxim Healthcare in Rockford is hiring for a Homecare Nurse to work 1:1 this summer starting May, 2025 through August, 2025 with a pediatric patient of ours in their own home in Pecatonica, IL. This patient is 14-years old and requires G-tube care. Paid training is offered!
Open shifts: weekdays, weekends 9AM-5PM (can be flexible)

Responsibilities:Utilizes the nursing process to plan, implement, and evaluate patient/client care and/or services provided to client(s) receiving contracted services, as requiredInitiates the plan of care when directed and requests from prescriber any necessary revisions as patient status and needs changeIncorporates therapeutic, preventive, and rehabilitative nursing actions based on patient diagnosis and established goalsIntervenes as applicable in response to changes in patient/client condition and reports such changes to appropriate physician or supervisorAdministers medications and treatments as prescribed, as well as, monitors and documents effectiveness of interventionReviews plan of care and program protocols and identifies areas for healthcare teaching/education, care coordination, goal setting and discharge planning needs, as applicable
Requirements:Current Licensed Practical Nurse (LPN) License for the state in which the nurse practices.Current Health Certificate (per facility Licensed Practical Nurse (LPN) contract or state regulation).Current PPD or Chest X-Ray.Current BLS card.One year prior Licensed Practical Nurse (LPN) experience preferred.
