Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations._ **Responsibilities:** GENERAL PURPOSE: Coordinates case management, rehabilitation therapy and family advocacy services for large 17-site ambulatory service with over 65,000 pt visits per year. Supervises six or more Rehabilitation Therapists, Senior Psychiatric Case Managers, Patient Navigators, and Peer Family Advocates. The position also has expertise in community resources and provides services directly to patients and their families as well. Serves as the Community Liaison of the service developing and nurturing relationships with regional community organizations (e.g., schools, churches, advocacy organizations). The position reports to the Director or Associate Director of the service. The person must subscribe to UR Medicine ICARE values, including, creating a culturally competent environment by treating patients, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences. Position hours: 8:00 am- 4:30 pm, Monday -Friday, closed on University holidays. **ESSENTIAL FUNCTIONS** + Collaborates with service leadership to design and implement case management, rehabilitation therapy, patient navigation, and peer family advocacy to meet the treatment goals of the patient population. Provides these services personally, as appropriate. Monitors service productivity and quality. Identifies areas for improvement and efficiency. + Trains and supervises Senior Psychiatric Case Managers, Rehabilitation Therapists, Patient Navigators, and Family Peer Advocates to work with primary clinicians and treatment team members to evaluate the needs of a patient and mobilize resources, (e.g., educational advocacy, food); attend school and community meetings as clinically indicated to meet treatment goals; and mobilize family and natural support networks when indicated. Oversees supervisees work with patients in acute crisis (e.g., collect information, prepare referrals, and follow-up on referrals to county based OMH services, Health Homes, Preventive Services, Residential Treatment Facilities (RTF) and Residential Treatment Centers (RTC).) In addition, oversees staff make timely collateral contacts for new and existing patients and families (e.g., Child Protective Caseworkers, Foster Care Caseworkers, other DSS Caseworkers, probation officers, pediatricians and nurse practitioners, and school personnel). + Serves as the Community Liaison of the service, UR Medicine: Pediatric Behavioral Health & Wellness Outpatient Services, Develops and nurtures relationships with regional and local community organizations (e.g., schools, churches, legal assistance agencies, educational advocacy organizations, child-serving organizations such as Boys and Girls Club, Recreation Centers, Kiwanis Club), to better understand the needs and resources of the Greater Rochester community and inform and collaborate with service and Division leadership on community initiatives. + Completes and maintains professional documentation.
