Supervised Independent Living Supervisor

Nexus-PATH is looking to add a Supervised Independent Living Supervisor to our team!

Position Summary: Provide oversight of Nexus-PATH’s Supervised Independent Living program (SIL), staff management, and leadership to ensure operational efficiency of the Program within all applicable rules and standards of the organization, as well as external regulating agencies.

Primary responsibilities:

Program oversight and development.

Review, correct, and approve all service entries for SIL clients. Assist in the planning of policies and procedures for the Supervised Independent Living Program; demonstrate the ability to plan, organize, and manage multiple priorities that make the best use of resources. Review, correct, and approve ANSA-T assessments and treatment plans. Collaborate and partner with the Social Services Coordinator to facilitate a compliant and seamless transition process for the client between Foster Care and Supervised Living. Provide comprehensive documentation within established timelines relating to intake services, needs assessment, case management, transition planning, and progress reports, to meet the needs of each client. Develop and implement a tailored transition care plan with goals and an action plan to support each client; leverage group services, case management, and service practice for successful client outcomes. Monitor and evaluate the client’s activities to ensure they are meeting required standards; thoroughly document and report concerns and reassess and adjust the transition plan as needed. Submit critical incident and sentient event reports while developing corrective action plans as needed. Serve as an advocate for the client; champion, support, and encourage each individual while demonstrating and teaching the client self-advocacy and self-sufficiency skills while in transition. Establish well-defined service parameters with legal custodian as the youth’s needs require. Leverage the ‘wraparound’ process by supporting the client in their home, school, and community to support the client while progressing through transitional living.

Community Partnership & Resource Allocation

Advocate for the development of funding and referral resources to meet the youth’s needs at the local, state, and national levels. Educate the community and community partners about Supervised Independent Living programs and the benefits of the programs with the intent of enhancing services for the SIL program and its clients.

Trauma Informed Care Practice Principles & Crisis Intervention

Monitor and assess the health and safety of the client’s living environment and living skills; provide support and guidance to the client in matters of independent living skills, routine, and discipline. Engage in efforts to strengthen the resilience of youth impacted by trauma; understand the potential for secondary traumatic stress, and practice self-care strategies to mitigate the youth’s risk. Provide crisis intervention to the client as needed including on-call crisis intervention. Routinely and proactively screen for trauma exposure and demonstrate proficiency by applying screening tool results to adjust and enhance services provided for the client’s independent living needs. Provides educational resources to youth, relating to trauma exposure and its impact.

Documentation, Caseload Maintenance & Productivity

Complete accurate and detailed documentation for Agency and governing/accrediting agencies; maintain current files on all individual clients in placement and/or in the SIL program. Ensure case documentation is thorough, accurate, and timely as required by standards. Produce reports as requested by parents, referral agencies, courts, etc.

Participate in and support the ongoing selection, evaluation, supervision, and development of staff.

Participate in recruiting, interviewing, and selecting qualified Independent Living staff, consistent with corporate policies and guidelines. Supervise assigned staff, monitor work progress, establish performance standards, and ensure that all performance evaluations are completed in accordance with Nexus standards; provide consultation and assistance as needed to ensure appropriate coaching and development of staff. Develop collaborative and cohesive teams through demonstrated leadership, problem-solving, remaining approachable, and communicating the Nexus mission, values, and shared goals and objectives. Encourage a climate of growth through planned development activities such as direct performance feedback and coaching, job rotation and shadowing, and continuing education

Required Education and Licensure:

Bachelor’s degree in social work, psychology, vocational rehabilitation, juvenile justice, education, child development, or related human service field. Proof of valid driver’s license, auto insurance and access to vehicle capable of safely transporting clients. Successful completion of background checks and all applicable health requirements. Must be 21 years of age or older. Valid driver’s license required. Must meet state regulating agency and Home Office driving requirements.

 Nice to Have: 

Minimum of two years experience working with youth to transition from adolescence to adulthood in the independent living program preferred OR two. or more years experience. 2+ years in a supervisory role

Travel:

Must be willing and able to travel within the community, city, or state level via air and ground transportation.  Use of a personal vehicle for regular local business travel will be required to support the clients and agency business offices or company-sponsored events may be required.
