ABOUT THE POSITION:CCSI is contracted by the Onondaga County Department of Children and Family Services (DCFS), Adult Mental Health to provide program management services, including specialized staffing. CCSI seeks a talented Supervisor for the System of Care (SOC) Outreach and Navigation Team to contribute their expertise in providing essential business services to our provider organizations and customers. As a member of the Adult Behavioral Health Outreach and Navigation Team, and partner of the Onondaga County DCFS, the Supervisor serves provides oversight and coaching support to the Team liaisons who serve as the point person for healthcare and human services organizations, law enforcement, social services and the individuals they refer who are identified as high-need, who experienced challenges related to substance use, mental health, criminal justice involvement and barriers to accessing services. This preventative response offers voluntary assistance to high need individuals in the community. The Supervisor is part of a multi-disciplinary service focused management team that assists with coordination of cross-system collaboration and planning to support and strengthen service navigation and outcomes. Requirements SPECIFIC RESPONSIBILITIES INCLUDE: + Reviewing, processing and assigning community-based referrals to team liaisons. + Provide supervision and coaching support for liaisons and oversight of the outreach and navigation process. + Consults with other team members, law enforcement, and community and organization provider staff on complex cases, issues and / or concerns. + Inputs and maintains records in the designated system within established timeframes. + Coordinates, participates in, and/ or facilitates/ co-facilitates meetings with community service agencies, professional and natural supports of referred individuals and government units as needed. + Participates in and assists with the facilitation of team meetings, supervisions, trainings and leadership meetings within the Adult Mental Health and Outreach and Navigation Teams. + Assist with the design, implementation and evaluation of the Outreach and Navigation Team by gathering, recording and analyzing program specific data. Creating and sharing reports on statistical program data and following through with quality improvement activities and planning. + Designing and delivering educational and professional development to the Outreach and Navigation Team. + Performs other duties as assigned. (CCSI Equivalency Chart (https://www.ccsi.org/CCSI/media/pdfs/Equivalency-chart.pdf) ) + Knowledge of community resources/providers. + Familiarity with mental health, medical, substance use and criminal justice terminology. + Excellent written and verbal communication and interpersonal skills. + Experience working with individuals with disabilities. + Individuals with lived experience in the mental health, substance use and/or criminal justice systems encouraged to apply. ADDITIONAL REQUIREMENTS: + Ability to satisfactorily complete required background screenings: criminal and driving. + Culturally responsive to the population being served in the development, design, monitoring, evaluation and/or implementation of programs and services. + Be able to work flexible hours to accommodate meeting schedules. + Respect the confidential nature of all CCSI and CCSI customer information you are exposed to in the course of your work performance.
