About the position: Unit Supervisor Do you enjoy working with vulnerable populations and making a difference in their lives? Are you passionate about individuals living with chronic medical, and behavioral health disorders? Are you ready to build a team of vibrant care managers as they carry out daily care coordination, health promotion, and referrals to community support services? Under the supervision of our clinical director and program manager, your essential functions include: •Direct supervision to care managers to assist with the development of effective clinical interventions and care plans •Ensure staff is kept informed of Health Home policies and procedures as well as departmental priorities. Receive appropriate trainings in order to maintain clinical standards •Provide crisis intervention to clients in distress •Complete enrollment and service date tracking reports monthly or as needed •Review and approve documentation as assigned •Consistently exhibit the values of team work, open book management, advocacy and customer service •Participate in quality assurance and quality improvement activities •Conduct staff meetings, trainings and supervision •Extensive field work is required to achieve client outcomes and meet health home requirements •Performs other duties as requested •Facilitation of bi weekly Clinical Case Review with the Care Managers What You Need •Bachelor's Degree Required; Master's Degree Preferred in Social Work, Psychology or Human Services. •Two (2) years post graduate experience working with individuals or families who have a history of mental illness, substance use, homelessness or HIV/AIDS •Excellent problem-solving, decision-making and judgment skills •Ability to work on an interdisciplinary team Your qualities: •Excellent interpersonal and time management skills. •Strong leadership ability •Ready to support and build a team of care managers who are eager to develop their skills •Able to adapt to new situations and settings •Able to use technology such as Microsoft Outlook, Microsoft Excel, among other programs/systems
