Responsibilities
Job Summary / Purpose
Provides leadership to Care Management RNs, Social Workers and support staff to accomplish the operational functions and performance goals of the department, which include efficient patient throughput, safe and effective care transitions, and compliance with payer and regulatory requirements.
Essential Key Job ResponsibilitiesOperational Efficiency Schedules team members so department is adequately staffed and patient needs are met.Monitors and adjust staffing to achieve productivity targets and accomplish essential work functions of department.Adjusts staff workloads to meet demand and ensure work is accomplished in a timely manner.Monitors expenses against budget and plans proactively for departmental costs and charges. Works with director to develop department budgets.Effectively uses managerial tools and software such as Infor, Kronos, Lawson, etc.Staff Performance Interviews and hires staff that are competent and will assist with meeting department goals.Trains and develops staff to ensure their work is of high quality and aligned with departmental, regulatory and payer requirements. Evaluates staff performance fairly and provides meaningful feedback on progress towards performance goals.Observes and coaches staff as they perform their work to give real-time feedback.Engages staff in department, hospital, and CHI Saint Joseph Health goals and results. Works with staff members to develop and achieve their own performance goals.Resource/Role ModelProvides expertise in problem-solving for difficult patient issues.Knowledgeable in all aspects of Care Coordination, Discharge Planning and Utilization Management, including CMS regulatory requirements, and ensures compliance. Provides supervision and coachingfor staff as applicable by professional discipline and licensure. Demonstrates excellent communication and negotiation skills. Required Education and Experience
Bachelor of Science in Nursing.
Will accept equivalent experience in lieu of degree if the candidate is enrolled in a BSN program and obtains his/her BSN within 2 years of hire.
Equivalent experience:
ADN – 3 years
BSN – 1 year
MSN – 0 years
Required Licensure and Certifications
Current unrestricted license as a Registered Nurse.
Required Minimum Knowledge, Skills, Abilities and Training
Clinical experience as Registered Nurse experience required:ADN – 3 yearsBSN – 1 yearMSN – no experience required.Minimum 1 year care management / utilization management experience required.Demonstrated knowledge in care management and utilization managementProficiency with electronic health record required.Proficiency with Microsoft Office (Word, Excel, etc.) required.Ability to understand payer requirements and payer communications related to care management and discharge planning. Ability to extract and manipulate meaningful data from various sources and develop data-driven action plans required.Excellent verbal communication and coaching skills required.
