Job Description:

The Clinical Program Supervisor is a supervising authority for the Check-In Program as part of the broader ElaraConnect Platform, and provides leadership, coordination, and supervision of teams and services in a safe, cost-effective manner. The incumbent is responsible for the generation of referrals for home care, both skilled and non-skilled, to improve patient access to our suite of services provided at Elara Caring, acting as a liaison between care providers and patients to ensure all home care needs are met appropriately.

What is Required?

High School Diploma or GED equivalent is requiredCurrent State License as a Registered Nurse4 years of Registered Nurse experience working in a home health, hospice, geriatrics, or general medical environmentPrior experience with surgical nursing or critical care, leadership, and HomeCare HomeBase EMR is preferredDemonstrates knowledge in all levels of home health nursing practiceDemonstrates strong leadership skillsDetail oriented with time management skillsReliable transportation to perform job tasks

You will report to the Director of Patience Services.
