Summary This is a Supervisory Health System Specialist/Director of Patient Centered Care and Cultural Transformation. The position is located in the Patient Centered Care and Cultural Transformation (PCC-CT) Service at the VA Salt Lake City Health Care System in Salt Lake City Utah. The incumbent will directly report to the Assistant Direct of VASLCHCS and be responsible for managing the implementation of Patient Centered Care (PCC) practices throughout the Health Care System (HCS). Responsibilities Duties include: Responsible for planning, preparing, and administering a budget that maximizes program strategies in a fiscally responsible manner. Coordinates closely with clinical and administrative Service Chiefs to align service goals and priorities with overall health system objectives focused on patient-centered care and the Veteran experience. Advises the Executive Leadership Team on personnel, services, supplies, equipment, and environmental changes or additions that would promote the culture of practice of PCC-CT Serve as the primary spokesperson for patient-centered care initiatives, representing the hospital to external stakeholders including other hospitals, medical schools, accrediting agencies, public health organizations, and the community. Craft and disseminate communications materials (press releases, media statements, talking points) to effectively convey the hospital's patient-centered care message and build relationships with key external partners and influencers. Formulate new policies and procedures aligned with patient-centered care principles, revise existing ones to support patient-centered care, draft correspondence, and reports as needed, and coordinate facility responses to recurring and special reports. Work closely with executive leadership to develop and implement strategic plans for advancing patient-centered care, collaborate with cross-functional teams to identify and address barriers, and provide input on budget development and allocation. Implements outreach initiatives that align with national and local strategic goals to establish trust, credibility, and rapport by providing valuable information, resources, and educational opportunities to Veterans empowering them with knowledge relevant to their needs and interests. Develop and implement a comprehensive strategy to enhance patient-centered care delivery across all VHASLC facilities, aligned with VHA mission values, goals, and critical success factors. Analyze existing and develop annual work plans outlining specific initiatives to advance patient centered care. Coordinates the development, implementation, evaluation and improvement processes and procedures to monitor the effectiveness, efficiency and productivity of services provided by the department. Develop and maintain relationship with external partners and organizations focused on patient centered care and cultural transformation. Serve as a liaison between VHASLC and other healthcare systems to develop and share Veteran experience and cultural experience best practices. Directly supervises a total of 11 full time equivalents consisting of the Veteran Experience Officer, MyHealtheVet Coordinator, and Patient Advocate Representatives. Makes decisions on personnel actions including selections, promotions, reassignments, retention, recognition, and adverse actions. Approves recommendations for incentive awards and rewards employees for good performance. Assigns and coordinates work based on priorities of the PCC program, Veteran Experience, and Office of Patient Centered Care and Cultural Transformation (OPCC&CT). Initiates interdisciplinary projects to improve organizational performance. Maintains position descriptions or functional statements. Delegates responsibility and authority to appropriate Program Manager Coordinator(s) and holds them responsible for the performance of their programs. Guides Program Manager/Coordinator(s) on problem solving, decision making, delegation, change management, confrontation management, and performance improvement management. Maintains a working knowledge of agency personnel management requirements as they relate to various employee groups and implementation of the PCC model. Performs other duties as assigned.
