This position is responsible for assisting with the oversight of the provision of support services, day to day programming and the support services staff in the Scattered Site program. This position assists with overseeing scattered sites Responsibilities • Responsible for assisting with the overall oversight of the Scattered Site programs in conjunction with the Reintegration Manager. • Assists the Reintegration Manager with the professional development of the support services staff upon hire and on an ongoing basis. • Assists with overall program development, implementation and oversight. • Oversees regulatory compliance and assesses the effectiveness of program processes, activities and individual case records, to ensure compliance with agency policy and licensing authority regulations. • Oversees the admission, intake, and orientation processes of new residents, as well as discharge planning processes. • Management of a resident caseload as needed. • Implements processes and works collaboratively with program staff to ensure Social Services benefits, other entitlements, and appropriate resident applications, re- certifications and fee collections occur. • Ensures general upkeep of scattered site properties in collaboration with the agency’s maintenance department. • May collaborate in the process of selecting new and/or replacement properties when required, with the Director of Operations and/or Property Manager. • Maintains on-call status for consultation and emergencies. May require attendance on- site for particular emergencies that occur outside of business hours. • Attends and participates in agency committees and special events. • Works in collaboration with the Reintegration Manager and Vocational Rehabilitation Supervisor to develop and implement the programs Vocational curriculum and ensures that program staff effectively implement this, along with provision of appropriate referrals. • Monitors and/or assists program staff with assisting residents with identifying, securing, and maintaining financial resources; effective budgeting; opening bank accounts; timely payment of rent; apartment furnishings; and other needs. • Assists program staff in identifying recovery, environmental, ADL, housing and vocational goals in collaboration with the residents to develop effective service plans. • Regularly completes required reporting. • Is responsible for ensuring support services are provided for all scattered site program residents. • Monitors and assists program staff with assisting residents with planning safe and sober activities, including recreational activities. • In collaboration with the Vocational Rehabilitation Supervisor maintains and up to date list of available community resources for recovery support, vocational, medical and housing needs that is provided to residents. • Oversees program staff responsibilities for assisting residents in maintaining linkages necessary to successful community integration. • Ensures program staff collaborate and coordinate services, maintain regular ongoing consultations with treatment providers, schools, legal, vocational agencies, and/or employers, etc. • Oversees purchases of program supplies and furnishings/appliances for residents. • Takes precautionary measures to limit transmission of communicable diseases (i.e., HIV, Hepatitis B, TB). • Carries out additional duties essential to the above-stated responsibilities as assigned. • Is mindful of and integrates Trauma-Informed Care and its five principles in all duties and interactions with fellow staff, residents, and external partners. Provides on- going training and modeling of Trauma- Informed practices to promote sustainability of practices. • Maintains compliance with all required rules and regulations including Federal Confidentiality (42 CFR, Part 2), HIPAA, Medicaid and/or HUD, and any others. • Regular attendance at work location. • Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. • Other duties may be assigned. Requirements Must possess and maintain an active QHP status (CASAC must be Masters or Advanced) and have a bachelor’s degree in Human Services or related field with a minimum of three years’ counseling experience. OR • Must possess and maintain an active QHP status (CASAC must be Masters or Advanced) and have an associate’s degree in Human Services or a related field with a minimum of five years’ counseling experience. • A valid New York State driver’s license is required. Salary Description 24.00/Hour
