Supported Work Instructor
Summary: Provide work training, supervision and support to individuals working at a supported work site. Essential Job Functions: + Transport Individuals in an Agency vehicle. + Act as mandated reporter if there is reasonable suspicion to suspect abuse or neglect of Individuals and make all required reports in established timeframes, as required by the N.Y.S. Protection of People with Special Needs Act. + Maintain all certifications required by CDS Monarch and state regulations. + Ensure the cleanliness of designated CDS Life Transitions buildings. + Respect all individuals + Provide meaningful and functional activity-based programming that is consistent with individual program/support plans, each Individual’s personal vision, and the mission and vision of the program for all assigned Individuals. + Support the individuals with choice-making + Get to know the individuals through assessment and discovery + Promote advocacy with the individuals + Facilitate personal growth and development with the individuals + Assist with the facilitation of supports and services for the individuals + Assist the individuals with building and maintaining professional relationships + Show respect for diversity and inclusion + Accurately communicate and document for individual program plans, reportable events, and incidents within established time periods. + Adhere to and promote the mission, culture, and practices of the organization + Adhere to corporate compliance policies and procedures + Know, understand, and follow the NADSP Code of Ethics + Support crisis prevention, intervention, and resolution by demonstrating skill in applying the principles and practices with behavioral management plans, positive approaches and SCIP-R and demonstrating respect for the safety of others. + Support health and wellness + Prevent, recognize, and report abuse. + Support the safety of all individuals including during environmental emergencies. + Follow proper safety procedures in transportation situations per policy and procedure. + Promote and ensure the dignity and rights of all Individuals, service providers, co-workers and customers. + Ensure a safe, organized, and clean environment that is conducive to learning. Inform supervisor of any related concerns. + Support community participation and contribution. Ensure necessary resources are available to complete scheduled activities. Provides support at community sites as needed. + Support the individual with employment, educational, and career goal attainment. + Monitor status of all adaptive equipment and individual belongings. + Provide oversight of cleaning operations of designated CDS LT buildings, individual cleaners, and actively participate in cleaning activities on a daily basis. + Complete work performance evaluation and time studies for assigned Associates. + Ability to accurately and contemporaneously track progress on goals in electronic platform on a daily basis. Ability to compile data to complete monthly summary. + Demonstrates appropriate work place etiquette in regards to but not limited to displaying a professional demeanor, dedicating attention to punctuality, attendance policies, reliability, flexibility, and effective communication, appropriate work apparel, co-workers, supervisors. + Ensures cleaning supplies are readily available at designated sites, and informs management of supplies that are needed. + Maintain contact with site managers as needed. + Review work assignments, progress and concerns/issues. Completes pre/post shift checklists of all assigned buildings. + Maintain all certifications required by agency and state regulations. + Attend meetings on and off-site as necessary. + Perform all other duties relevant to the position as assigned by supervisor. Knowledge, Skills, and Abilities + Must Meet CDS Monarch vehicle operator requirements. + Able to work independently and motivate others. + Able to communicate effectively, both orally and in writing. + Maintain all required certification/training by state regulations and CDS Monarch policies. + Able to read and write English at a level of fluency and comprehension which enables the candidate to follow written directions on labels, charts, medical devices and other sources of client care and/or production information. Must be able to follow, comprehend and appropriately act upon same if transmitted orally. Education and Experience: + High School diploma or Equivalency (GED). + Must be at least 18 years of age. Physical Requirements/Working Conditions: + LIFTING: Supported Work Instructor may frequently lift 0-25 lbs., occasionally lift 26-50 lbs., rarely lift over 50 lbs. + PUSH/PULL: Supported Work Instructor may frequently push/pull 0-25 lbs. of force, occasionally push/pull 26-50 lbs. of force, and rarely push/pull over 50 lbs. + SITTING: Supported Work Instructor may sit 0-10% of the working day. + STANDING: Supported Work Instructor may stand 30-40% of the working day. + WALKING: Supported Work Instructor may walk 50-60% of the working day. + STOOPING: Supported Work Instructor may frequently stoop. This is defined as bending body downward by bending spine at the waist. + KNEELING: Supported Work Instructor may occasionally kneel. This is defined as bending legs at knees to come to rest on knee(s). Supported Work Instructor must be able to kneel in emergency situations to administer CPR. + CROUCHING: Supported Work Instructor may occasionally crouch. This is defined as bending body downward and forward by bending spine and legs.
