Home Base: Bowie, Maryland
Areas Needing Service: DC, MD, VA
Schedule: Full Time, M-F, 9a-5p, 40hrs per week
Additional Schedule Details: HYBRID SCHEDULE – Office 1 day per week, other days will be visiting clients at home (no facilities) or working remotely from home
Commuting Details: Range is normally under 50 miles from home office but could be less than 2 hours max from office. They will plan on keeping staff in one general area.
Duties
Staff will be visiting client’s homes doing assessments, monitoring plans, doing care coordination, etc.
Majority of the days will be at client’s homes but they will come into the office 1-2 times per month
Case Load: Roughly 20-30 total – they will coordinate the assessments/check-ins accordingly
Requirements
No license/certs required
Staff only need a minimum of a Bachelor’s in a healthcare / human services (Healthcare Administration, Psych, Social Work. Etc.) or related field
They must also have least 1+ years of experience in a case manager/support planner role (they would consider someone with 6+ months of experience)
The Caseworker is responsible for helping adults, childrenand families find and obtain government resources like healthcare services,financial aid or counseling. Dutiesinclude meeting with clients to identify their needs, using resources in thegovernment, healthcare and financial service industries to offer assistance totheir clients/patients.
Minimum Requirements:
