SUMMARY

Team Case Assistant coordinates patient services under the direction of the team manager/designee. This includes but not limited to prioritization and coordination of daily patient schedules, requesting additional services such as aide, therapy, social work, interpreter services, and ensuring that referrals for services are appropriately assigned in a timely fashion.

STATUS: Full Time          

LOCATION: 490 East Avenue        

DEPARTMENT: ElderOne

SCHEDULE: Weekdays 9am-5pm, and every other weekend 2pm-10pm

Ability to multi-task in a fast paced environmentProficient use of Microsoft office.One year of experience in home care setting or medical office preferred.For ElderONE only: 1 year of experience working with a frail or elderly population preferred

RESPONSIBILITIES

Customer service. Manages multiple phone calls in a timely and professional manner providing appropriate follow up to assure callers’ needs are met. Checks email in a timely manner providing follow up to assure sender’s needs are attended appropriately. Provides exemplary customer service. Responsible for coordinating participant services and staff scheduling. Assures appropriate routing of calls and assistance with transitioning participants through care. Team Support. Provides field staff assistance and resources for appropriate day to day issues as they arise under the direction of the team manager/designee. Builds and maintains strong, communicative relationships with the team. Performs general office duties/projects including but not limited answering phones, documentation/order processing, copying, filing, mailing, and obtaining/ordering office supplies. Assists in orientation of new staff, role modeling, participates in quality assurance activities. Attends team meetings and other internal meetings as requested. Assures complete and accurate schedules for ElderONE staff, primarily HHA's and Nurses. Provides a range of supportive services to Home Health Aide operations of ElderONE.Process Management. Collects, organizes and reviews various programs for information and reporting and initiates follow up.  Updates the EMR per agency standards. Performs functions related to transfers or discharges as required by regulation. Creates and schedules case assignments, and assists with timekeeping as needed.

S - Sedentary Work - Exerting up to 10 pounds of force occasionally Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
