be highly organized able to multi task and have strong computer skills This role will collaborate with all team members to achieve exceptional outcomesJob DutiesCollaboration with peers across the regionsProcess improvement with providers and suppliersCommunication with referral sources vendors patients and all team membersEMR updates as necessaryReports to primary nurse supervisor physician social worker or therapist regarding casesConsults with physician regarding client needs and plan of care with pharmacies DMEDirects patient complaintsissues to appropriate personnelParticipates on agency committees and special projectsAttends and participates in in service education programParticipates in Team meetings and case conferences LEAN Daily Management process including huddles scorecards Visual Boards IDTUtilizes data management systems to manage patient documentation calculates and analyzes data from reports to ensure timely accurate compliance and month closeMaintains knowledge of and compliance with Federal State and accreditation regulations as well as Agency policies and proceduresSeeks consultation and guidance from supervisorMaintains productivity standardCoordinates agency service delivery and solves logistical problems for customers and clientsProvides client information to physicianrsquos offices and receives care plan changes via verbal ordersDocuments specific patient information including verbal orders from physicians transports verbal orders from physicianrsquos offices as neededIntervenes via telephone andor through the application of tele home care technology for patients communicates changes in health status of patients to appropriate cliniciansRefers callers to appropriate community resources or agency services when neededReceives and relays information between field staff physicians and other resourcesParticipates in quality assurance and quality improvement activitiesWork where every moment matters Every
