GENERAL SUMMARY:
Under minimal supervision, Team Lead- HHC Intake Therapy is responsible for:

Case load/referral management and the overall activities of a specific Henry Ford at Home team. Utilizes the knowledge and application of therapy process and principles to coordinate the provision of high-quality, cost-effective care. 

EDUCATION REQUIRED: 

Requires a minimum of a Bachelor's degree in Physical or Occupational Therapy from an accredited school. 

LICENSE REQUIRED:

Requires current licensure in the State of Michigan as a Physical or Occupational Therapist.

EXPERIENCE REQUIRED:

Requires a minimum of one (1) year of progressively responsible home health care therapy experience. Progressive experience in program development and coordination preferred.
