Provides career coaching, wraparound support and essential skill development for newly hired team members of Advocate Aurora Health. Supports the identification and addressment of barriers to employment retention in newly hired team members; specifically those of diverse and underrepresented communities that experience higher barriers to employment and retention. Addresses barriers to employment by providing appropriate wraparound supports and intervention. Maintains a rotating caseload of newly hired team members.
Major Responsibilities:
Maintains frequent contact with an active caseload of newly hired team members across multiple sites and conducts regular one-on-one sessions both in-person and virtually.Assists team members in creating employment plans and identifying barriers to employment goals. Motivates and guides team members in meeting the goals laid out in their employment plans.Maintains accurate and timely case documentation.Conducts assessments, establishes appropriate plans and initiates job coaching and wraparound support plans. Develops processes for assessment and coaching in partnership with Program Manager.Provides coaching support for team members in emergent or crisis situations and connects to internal or external resources.Provides education to team members on career growth and development, essential skills, educational assistance, post-secondary education, work-based learning and other career navigation services.Assesses need and allocates financial and wraparound support as needed (e.g., transportation, gift cards, child care) in partnership with Program ManagerPartners with community-based organizations, workforce-investment boards, social service support agencies and other community resource providers to link team members with external supports.Assists in the execution of various workforce development programs, events and functions as required in partnership with the Workforce Development team.Connects team members to financial wellness, banking, tax and financial support entities
