Overview

The Triage LPN - SNF provides On-Call and Triage for the assigned population in accordance with triage protocols. The tele-triage LPN assess health status, identifies problems, and needs of person(s) served in collaboration with other LPNs, facilities, and providers. You will handle incoming phone calls and perform outbound contact based on individual need.

 

SNF Traige LPN (Fully Remote) 

Full Time: 2 position available immediately

 

Updated Available Schedule:

1)Friday-Sunday 10am-6pm CST 

2) Friday-Sunday 6am-2pm CST 

Responsibilities Monitor and handle inbound phone lines during service hours to ensure timely response for caller needsTriage all symptom-based calls and give recommendations according to approved triage protocolTriage patients by assessing severity of symptoms and collaborate with facility/providers to determine appropriate careDocument in real time all care related interventions and recommendations to ensure local operations are notifiedCommunicate with facilities and providers via EMR system and phone as necessaryEnsure compliance with state/local requirements through proper documentationParticipate in performance improvement activities and maintain ongoing clinical knowledgeNotify facility and providers of all encounters regarding patientsActively participate in applicable meetings as requested Qualifications Two years of experience in a clinical healthcare setting High school diploma/ GED required; Associate’s Degree or some college preferredLicensed Practical Nurse (LPN) with an active license; compact license preferred.Experience with electronic medical record software and proficient use of the Microsoft Office suiteAbility to multitask, prioritize, and manage multiple projects simultaneouslyExcellent written and verbal communication skills and ability to work independently or in a team setting About our Line of Business Starting Point Nursing Services is staffed 100% by a team of remote Registered Nurses trained to support the Post-Acute Continuum with clinical advice and interventions. Our services are tailored to our partners, including remote physical/chart assessments, predictive care modeling, clinical programming interventions for high-risk populations, emergent care coordination, and case management.
