Our Company Starting Point Nursing Services Overview The Transitional Care Tele Triage LPN provides services to the assigned population in accordance with protocols. The transitional primary care LPN serves as a resource for telehealth patients and providers. You will serve as a coordinator for this virtual visit program. Responsibilities + Schedule needed virtual visits in coordination with telehealth provider + Receive and upload records into HCHB, starting client chart + Send RingCentral link to designated HH clinician and client for scheduled visit + Ensure compliance with state/local requirements through proper documentation + Participate in performance improvement activities and maintain ongoing clinical knowledge + Document in real time + Actively participate in applicable meetings as requested + May complete SNF Triage LPN duties as needed Qualifications + 2 years of experience in a clinical healthcare setting + High school diploma/ GED required; Associate’s Degree or some college preferred + Licensed Practical Nurse (LPN) with an active license; compact license preferred. + Experience with electronic medical record software and proficient use of the Microsoft Office suite + Ability to multitask, prioritize, and manage multiple projects simultaneously + Excellent written and verbal communication skills and ability to work independently or in a team setting About our Line of Business Starting Point Nursing Services is staffed 100% by a team of remote Registered Nurses trained to support the Post-Acute Continuum with clinical advice and interventions.
