Telehealth Pharmacist
Job Title: Telehealth Pharmacist Job Description The Telehealth Pharmacist will provide high-touch telehealth services including Medication Therapy Management (MTM), basic medication education, referrals to support services, and more. This role focuses on patient-centered care delivered remotely through various telehealth platforms. Responsibilities + Counsel participants on the importance of medication adherence, including alerting them to missed dosages and refills. + Provide medication education and self-care education for specific chronic conditions. + Contact physicians to schedule diagnostic tests and make medication therapy recommendations. + Connect with and support Community Health Workers advocating for the participant. + Connect participants with community-based resources as needed. + Assess a participant’s health status and develop a medication treatment plan. + Monitor and evaluate a participant’s response to therapy. + Provide comprehensive medication reviews to identify, resolve, and prevent medication-related problems. + Document the care provided and communicate essential information to a participant’s primary care providers. + Provide oral education and training to enhance participant understanding and appropriate use of medications. + Provide information, support services, and resources to strengthen participant adherence to therapeutic regimens. + Coordinate and integrate MTM services within a participant’s broader health care services, caregivers, and advocates. + Maintain proper documentation of each service provided to patients, including patient information, service details, and treatment plans. + Coordinate the scheduling of MTM-related services and patient signatures directly after service. + Coordinate and communicate with in-store pharmacy staff about patient-related topics. Essential Skills + Pharmacist (PharmD is strongly preferred) + MTS Certification + Telehealth experience + Medication Therapy Management experience + Experience with Medicaid patients + Schedule flexibility Monday through Friday, some weekends Work Environment The Telehealth Pharmacist must work a 45-hour work week with the flexibility to create their schedule Monday through Sunday, ensuring the completion of hours by the end of the week. The pharmacist will spend one hour with each patient and will schedule these calls independently. The candidate must reside in the state of Missouri. Pay Rate: $50-55/hr Pay and Benefits The pay range for this position is $50.00 - $55.00/hr.
