Our multi-disciplined treatment team works with individuals ages 18+ who experience severe and persistent mental illness, co-occurring developmental disabilities and substance use disorders. The Adult Therapist provides diagnostic assessment, treatment planning, therapy, and crisis management. This is a full time, 40 hour per week position. This is a hybrid position, living locally within our facility is required to be able to serve our community. Major Functional Areas and Description of Duties + Provide individual diagnostic assessment to determine client’s strengths, needs, resources, and objectives; + Formulate treatment plans specifying presenting problems, objectives, interventions, and desired outcomes; + Complete assessments for eligibility determination for State funded services; + Provide individual and group therapy in the office and/or remote via telehealth; + Provide crisis management and intervention when clients experience acute exacerbation of symptoms to ensure their safety within the home and/or community; + Work collaboratively with other treatment team members, natural supports, and external providers to ensure a wrap-around approach is provided for continuity of client care; + Provide timely documentation on each service delivered; + Provide set hours of billable service each week (eligible for incentive pay upon exceeding productivity expectation); + Engage in appropriate consultation meetings and committees, both internal and external, as determined by supervisor; + Function professionally and in a manner that protects the integrity, confidentiality and rights of all patients; + Execute all required services and documentation in accordance with agency policies and federal/state standards; + Maintain professional memberships, licenses, accreditation, certifications, etc.; + Represent CLM in a professional and appropriate manner in all settings; + Perform all other duties as assigned by supervisor. Qualifications and Skills + Master’s degree in mental health field required (social work, clinical mental health counseling); + LCMHC or LICSW preferred; + Experience working with co-occurring disorders preferred; + Ability to exercise sound clinical judgment in routine, urgent, and emergent clinical situations; + Proficient computer skills, specifically with all Microsoft office applications; + Ability to document effectively and efficiently in course of clinical work; + Ability to navigate an electronic health record; + Ability and desire to work as part of a clinical team while also able to be self-directed and motivated with minimal supervision; + Ability to acquire and effectively practice new clinical skills as needed according to evolving agency needs/services; + Effective communication skills, organizational skills, and ability to multi-task. Physical Demands and Working Conditions A majority of the work will be in the office and/or remote via telehealth. Manual dexterity is needed to use computers. The ability to sit for extended periods of time is required. Reliable internet access, transportation, a clean driving record, and proof of auto insurance coverage is required.
