PURPOSE: To provide therapy and behavioral planning for children and some adults who have developmental, behavioral and/or emotional disabilities and their families for Little City’s InHome and Family Services’ Foster Care & Adoption and Home-Based Services programs.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
The list of essential duties and responsibilities, as outlined herein, is intended to be representative
of the task to be performed. The omission of an essential function does not preclude management from assigning duties not listed herein is such duties are a logical assignment to the position.
1. Carries a caseload of clients and families focusing on the use of individual, group or family treatment.
2. Assesses clients using Rule 132 and Rule 140 Medicaid standards and completes the appropriate assessment authorizing services.
3. Develops the treatment plan, per Rule 132 and Rule 140 standards and routinely evaluates the individual’s success in treatment
4. Attend or request staffings, as necessary, as well as attend any other meetings related to the
child for the purpose of coordinating interventions with the interdisciplinary team including testimony in court.
5. Provide consultation with other program staff, biological parents, foster parents, etc. regarding children’s therapeutic issues as needed.
6. Documents contact with client, client’s family (clinical documentation requirements) according to Medicaid Part 132/140, agency, DCFS, and DHS standards.
7. Attend and participate in individual and group clinical supervision on a weekly basis.
8. Attend and participate in team and department meetings as scheduled.
9. Maintain and increase clinical skills through training and research.
10. Provide training to families, staff and other service providers.
11. Provide on-call crisis intervention and support on a rotating basis.
12. Maintain an awareness of all safety activities in the area and immediately report all accidents
and safety hazards to supervisor.
13. Maintains a current American Heart Association certification in First Aid and CPR.
14. Successfully completes Little City Foundation required training, re-training, and any additional training as required by Little City Foundation policies and practices, or as assigned by his or
her immediate supervisor.
15. Other duties as assigned by supervisor.
