Position Summary:

The person in this position will be responsible for conducting in-home/on-site assessments for adolescents and provides individual and family therapy.

 

Essential Job Functions:

Monitor the clinical systems, procedures, and standards of the program, informing the appropriate parties when deviations occur Conduct and/or Conduct and/or revise SMQ-10 assessments conducted at the time of consumers admission, and/or at the time of consumers referral to a higher level of care Review Interpretive Summaries to ensure they are based on information obtained from the assessment Review Wellness and Recovery Plans with the family therapists. Review monthly Wellness and Recovery Plan Reviews Document all clinical services and activities in accord with licensing, Medicaid, and Joint Commission standards and agency policy and procedure Review clinical records to ensure compliance licensing, Medicaid, and Joint Commission standards and agency policy and procedure. Provide in-home/on-site individual therapy, as needed. Provide in-home/on-site family therapy, as needed. Provides therapy to additional populations, as needed. Participate in case conference Prepare Discharge Summaries, as needed.
