The incumbent of this position provides quality clinical services on behalf of Adult & Child across lifespan of clients in the school based, home and community settings based on client needs.

Essential Job Functions:
Providing clinical services and evidence based/best treatment to assigned caseload:
Assigned caseload could include coverage across service line for treatment including, but not limited to, assessment, therapy, skills development, and case management services.
Provide crisis intervention/emergency services:
By assessing risk, development of safety plans, reporting of any abuse or neglect, coordinating hospitalization or any other emergency situations.
Communication and collaboration with collateral contacts:
By consulting with providers regarding integrated health, current function, medication related- issues, educations support, and any other care related to client/families.
Develop and maintain effective relationships:
By maintaining open and professional communication with Team Leaders, clients/families, other consumers/constituencies, referral sources, staff/team members
Accountability and Documentation:
By completing documentation timely and accurately, maintaining ethical standards of Adult & Child, maintaining proficiency in the EHR
Service Delivery Expectations:
By meeting the identified service delivery expectation for your position

By promoting open communication and building upon the strengths of the team

Service Delivery: by meeting the expected position percentage by pay period and YTD
Notes Timeliness: By submitting notes within 48 hours after service 95% of the time
Documentation Timeliness: Submitting all other documentation to meet timeliness standards
Working Towards Licensure: By attending supervision 2x/month

Experience and Education Qualifications:
Master’s Degree in Social Work or related field.
Must be license eligible (LCSW, LMHC, LMFT, etc.)

Employment Requirements:
Successful completion of background check including criminal record, driving record, abuse/neglect and fingerprint check.
Completion of New Hire Orientation at the beginning of employment.
All training requirements including Relias at the beginning of employment and annually thereafter.
Current driver’s license, acceptable driving record and current auto insurance.
Physical Requirements:
ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally (exists up the 1/3 of the time) and/or a negligible amount of force frequently (exists 1/3 to 2/3 of the time) to lift, carry, push, or pull, or otherwise move objects, including the human body. Repetitive movements of hands, fingers, and arms for typing and/or writing during work shift.
Sedentary work involves sitting most of the time but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally, and all other sedentary criteria are met.
