Status: Exempt Reports to: Transition Coordinator Supervisor
Territory: Hamilton and surrounding counties

We are seeking a Transition Coordinator who will share in our vision to advance better living and care for seniors!

Qualified candidates for this position offer:

Education: High school diploma or GED required. Bachelor’s Degree in social work or a related field preferred.Experience: Minimum of 1 years in social service delivery with the elderly and/or family populations. 2-4 years of experience preferred. Demonstrated working knowledge of supportive services and other resources in the area served by the project.Travel: Frequent, necessary on a daily basis.

ESSENTIAL FUNCTIONS
• Serves as a liaison between the health care and housing professionals, including but not limited to the service coordinators, discharge planners, home/community based service representatives. Is knowledgeable on all service lines including adult day health, personal care/private duty, service coordination, skilled care and hospice. Works closely with all managers of all service lines.

• Works closely with individual case manager to assist individual in arranging services based on community resources for the individual’s needs and preferences such as physician, pharmacy, etc.
• Assists the individual with locating housing. Purchases materials and supplies for living.
• Assists on behalf of individual to ensure receipt of entitled benefits.

• Provides quality customer service to all residents to answer questions, address concerns, problem solve, assist, and provide results for the individual with their needs on a daily basis.
