Transition Coordinator II

As a Transition/Care Coordinator II you will be responsible for the delivery of quality psycho-social care to the youth and families that we serve. This position works with Clinicians and other care providers to ensure that overall needs of the client are met. 

This position is full-time, Monday-Friday, 9:00am-5:00pm, with some evening and weekend hours occasionally required as needed.
 

Responsibilities:

Works as a team member in assessing a youth’s needs and is responsible for developing assessing and modifying the individual service plan in collaboration with the child/youth/family and multi-disciplinary treatment team. Actively involved in the implementation of a strength-based service plan including site/home visit and assessments, identifying and coordinating collateral resources, recognizing gaps and needs, assessing results and communicating with the multi-disciplinary treatment team. Participate in agency and team meetings, volunteering and serving on agency community committees as needed. Provides therapeutic support, case management, coordination and linkage to services for youth discharged from the RTF. Utilizes experience, education, age-specific knowledge and cultural competence in order to assess and support youth and families to achieve positive and successful outcomes and routinely successfully used problem-solving skills with self and others.

Requirements:

Bachelor’s degree in social work, psychology, sociology or a related human services area of study and at least 4 years of experience working with severely emotionally disturbed children or Master’s degree and at least 2 years of experience working with severely emotionally disturbed children. Must have thorough knowledge of the services provided, well developed advocacy skills, and an understanding of payment processes for services Must have a valid New York State Driver's License and meet agency driving and insurance standards

$20.18 Minimum Rate per hour, $30.28 Maximum Rate per hour, based on experience.
