What you’ll do

We will look to you to represent our home health and hospice locations and our organization to the greater community. You’ll work with case managers, social workers, and discharge planners to generate and facilitate new referrals. Patients and families will look to you also to help evaluate their needs for home health or hospice services.

 

What it takes

Bachelor’s degree in business, marketing or nursing preferred; orTwo years’ experience in a hospital setting required Strong knowledge of the full continuum of care and eligibility requirements for home health, hospice, skilled nursing, and assisted living requiredMust possess a valid driver’s license and acceptable driving record Location Information
