BAYADA Home Health Care, in partnership with Jefferson Health, is currently seeking a **Transitional Care Manager (TCM)** who will be **stationed at Jefferson Einstein Hospital in Philadelphia, PA** . TCM's at BAYADA interact with discharge planners, case managers, and clients and families at designated facilities to coordinate the initiation of home health care services.
