SPECIFIC RESPONSIBILITIES:
· Conduct task analysis to determine job duties and communicate these to students.
· Travel Training as necessary to increase independence
· Assist students with preparing for NYS drivers permit exam and driver’s license in addition to registering with the Selective Service Office and voters registration
· Communicate with student about specific work/social skills needed for particular employment situations.
· Develop and strategize Career Plans with students (CDOS)
· Conduct (NYSED) Careerzone assessments and career interest surveys as directed.
· Train and support student in job responsibilities and tasks.
· Collect data to assess student job performance, and interactions with co-workers as well as supervisors.
· Assist student with developing strategies for budgeting and banking skills.
· Promote and foster the student with developing their independence, confidence, and self-advocacy skills in the work place with a goal of unsupported employment.
· Teach career and exploration classes
· Develop Employability profile (CDOS)
· Meet with students, families and staff to perform Level I Assessments.
· Develop, update and expand post-secondary plans, goals and transition needs for students IEP.
· Participate in CSE meetings
· Conduct college exploration sessions and visits.
· Conduct college planning meetings with students and assist them through college application process
· Instruct and oversee the development of student resumes.
· Consult with educational staff in regard to aligning lessons with careers. (CDOS)
· Pursue continuous professional growth.
· Pursue community internship opportunities for students with organizations and businesses.
· Participate in, coordinate and conduct activities that contribute to the effective operation of the transition process. Including meetings with parents, educational staff and clinical team.
· Update and maintain required state mandates and documentation in student file.
· Organize, interpret and administer interest and achievement surveys for students.
· Educate and inform parents and students of outside post-secondary adult agencies and services, in addition to information on guardianship, SSI, and OPWDD
II. QUALIFICATIONS:
· BA/BS required, Masters preferred.
· Prior experience with job coaching/career planning a must.
· Experience and/or degree in computer programming a plus.
· Proficiency with Microsoft Word and Excel
· Attending TCI class for certification/re-certification and passing a test upon completion of course is a minimum requirement for employment.

IV. WORKING CONDITIONS:
Therapeutic residential school. The Transition Counselor is afforded the opportunity to have supervision sessions with the Clinical Coordinator in order to provide opportunity for growth.
