Transplant Coordinator - Kidney Acquisition
Overview Fairview Health Services is looking Transplant Coordinator to work on the University of Minnesota Medical Center Campus in Minneapolis, MN. This is a casual position, meaning all hours are as needed. Responsibilities Job Description The Transplant Coordinator is responsible for coordination and preparation of patients for organ transplantation and the long-term management of patients after transplant through collaboration with various members of the interdisciplinary team. Collaborate with interdisciplinary team members within the organization and outside community partners Act as a liaison for transplant physicians to implement medical management of conditions Provide telephone triage and make patient care decisions based on patient symptom assessment and evaluation, laboratory results, and patient condition changes Apply appropriate medical interventions and transplant protocols based on patient assessment, laboratory results, and patient condition changes Assumes on-call responsibilities after office hours by providing a resource for medical triage. Provides on-going education to patients and families Collaborate with other members of the clinical team to improve patient care and benchmarks through quality improvement projects Participate in the development and revision of protocols Maintain knowledge of current practices in transplantation including regulatory requirements, nursing, and healthcare Qualifications Required BA/BS in Nursing 3-5 years acute care experience in ICU, CCU, Transplant, Acute Care Med-Surg MN RN license BLS EEO Statement EEO/AA Employer/Vet/Disabled: All qualified applicants will receive consideration without regard to any lawfully protected status Benefit Overview
