A Transportation Specialist:
Provides transportation for senior residents in a community van, bus or car to medical appointments and other outings. Escorts and checks residents in at medical appointments, handling medical document delivery, receipt, and communication as needed. When not driving this position participates in resident focused activities.Overview

Responsibilities

Provides assisted transportation for residents in a safe and caring manner. Destinations include medical and dental appointments, banking, post office, weekly outings, shopping for residents and staff and misc. Provides shuttle/courier service as needed. Coordinates transportation by car, bus, or van, scheduling to best meet resident needs for delivery and pick-up from appointments. Keeps supervisor informed of daily schedules, changing needs, and functions in a timely manner.Medical Appointment Support
Escorts and checks residents in to medical appointments as needed. Verifies that medical documents accompany resident to physician's office and may handle this task for resident. Verifies that appropriate medical documents are returned to the care team upon completion of a medical visit. Exhibits good follow through behavior in all transactions.Interpersonal Skills
Establishes and maintains good interpersonal relationship with residents, other staff members, and outside professionals while assuring confidentiality of resident information.Logbook Maintenance
Keeps record of time for billing purposes in Driver's Logbook. Maintains cleanliness of all vehicles used. Completes Driver Vehicle Inspection Report for any maintenance problems and reports condition to Supervisor.Resident Focused Activities
While not driving, participates in resident focused activities such as walking and talking with a resident, reading to residents, or assisting with other planned activities.Equipment Maintenance
Maintains 2-way radio, pager, and cellular phone equipment that is provided for this position. Ensures that equipment is in good working order and coordinates any repairs with Supervisor in a timely fashion.Resident Wellness Reporting
Reports changes in residents' physical or psychosocial functions as appropriate to the Wellness Director.Qualifications – Position Requirements
At least 18 years of age. Cleared Criminal Record clearance through the Department of Social Services prior to presence in the community. Health screening and TB test results not older than 6 months. Current First Aid Training and Certifications as per Title 22. Demonstrated capability to collaborate effectively within a team setting. Possession of a valid driver's license along with a clean driving record is required.Skills and Attribute
Sensitivity and knowledge of working with older adults and the aging process. Excellent written and verbal communication skills.Must understand health care environment and the medical context for older adults. Ability to communicate professionally with residents, families, peers and employees in a multi-cultural environment.
