The Trauma Clinical Nurse Navigator Serves as the coordinator for the care of patients in their specific specialties. Responsible for providing support, education, referrals, coordination of care, and other assistance to the patient and their family before, during, and after their healthcare experience.

 

Patient Coordination

 

Coordinate with the unit Patient Care Leader (PCL), case managers, social workers and on complex casesAssist with discharge planning, coordinating with patient, family, social work, case management and the trauma surgery team Liaison with financial assistance for unfunded/underfunded patientsAssist with identifying patients and families that have a need for psychological services and liaison with psychological services teams for inpatient and outpatient servicesCoordinate out-patient follow up for testing, scheduling with providers and other outpatient services as needed Assure that patients and families understand discharge plans and resources available including primary care optionsComplete Brain and Spinal Cord Injury Program (BSCIP) referralsRound with Surgeon or Advanced Practice Provider (APP) as requestedPerform follow up communications with patients and/or familiesRespond to Navigator consultsOversee the implementation and management of evidence-based practice guidelines

Administrative

 

Attend monthly multi-disciplinary Trauma Systems and Operations Committee meetings and provide updates as appropriateAssist with guideline/policy updates as necessaryCommunicate operational barriers to trauma services and hospital leadershipPerform chart reviews as requestedAssists with preparation and presentation for trauma site surveys

Education

 

Collaborate with hospital-based education teams to keep staff updated on policy and evidence-based practice updatesReturn patient phone calls and assist with patient education

 

 

Requirements:

RN (Registered Nurse) LicenseAn Bachelor Degree in Nursing is required2 years acute care experience as a Registered Nurse Certification in Related Specialty must be obtained within the first 6 months if not current
