Trauma Outreach Specialist

Work Shift:

Position Overview:

Under the direction of the Program Coordinator, responsible for the provision of intensive case management and peer mentoring services provided by a violence prevention/intervention grant program that provides bedside visits and intensive home-based follow-up services to a target population who have been hospitalized for violent injuries at Capital Health. In coordination with leadership, develops, implements, evaluates and improves policies, programs and services consistent with the organizational strategic mission.

MINIMUM REQUIREMENTS

Education:

High School Diploma or equivalency. Associates degree preferred.

Experience:

Applicants with personal experience in overcoming violence/ violence-related injuries/ the criminal justice system are encouraged to apply. Ideal candidate would have lived experience.

Other Credentials:

AHA BLS. Non-Licensed must obtain before end of orientation period

Knowledge and Skills:

Possesses a wide array of innate abilities, honed skills, and specific skill sets for successful interventions to take place including strong case management experience. Strong communication skills, writing reports, maintaining records, giving verbal case updates, and advocating for clients. Knowledge of urban violence issues. Must be punctual and extremely reliable. Highly organized and detail-oriented. The expectation of this position requires full-time on-site work and flexibility to work some evenings and weekends.

Special Training:

Proficiency with MS Office: Excel, PowerPoint, Outlook, Word.

Demonstrated commitment to working with target population. Ability to take constructive criticism and work well with supervision. Ability to maintain poise and professional courtesy in stressful situations and to work well with diverse populations. Capable of managing detailed tasks and apply critical thinking skills. Demonstrated ability to work independently and as part of a team. Ability to perform assessments with inpatient and emergency room trauma patients. Must be able to present self and program in a professional manner.
