TREATMENT COORDINATOR

Hourly Pay Range: $23-$24.41

The treatment coordinator is responsible for the coordination of clinical services to all individuals receiving services from the People Inc Article 16 Clinic and its 10 certified satellite Buffalo locations, as well as the satellite locations in the Rochester Area. Prepares for and participates in DDSO A16 surveys with senior treatment coordinator. Participates in treatment/program planning and maintains appropriate records, statistical reports, etc. so as to ensure appropriate treatment activity as well as maintain program efficacy.  Ensure that confidentiality is strictly maintained concerning all agency related information.

ESSENTIAL FUNCTIONS/RESPONSIBILITIES:

• Ensures the admission/intake process procedures are followed and the required paperwork is completed. Ensure service requested is not currently being provided elsewhere and the referral form is reviewed and signed off by the medical director. During intake, review with the potential service recipient and/or collateral as to the People Inc Art 16 procedures, rights of consumers, HIPPA etc., ensure consents are signed and scanned into chart along with intake packet. Complete intake note electronically and send to Medical Director for review and signature.
• Provide the clinicians a schedule of plans due. Follow up as to the current status of treatment plans and progress as well as appropriate modifications of the treatment plan. Send out treatment plans, semi annuals and significant changes to MSC’s annually, semi-annually and when plan changes. Follow up with any lapse in completing plans in a timely manner with appropriate personnel including billing if necessary to ensure appropriate resolution.
Communicate with person’s supports and providers to ensure documentation accurately reflects services being provided. This would include treatment plans Semi annuals, ISP’s. Ensure that the patient is receiving the service(s) as scheduled/recommended.
• Follow-up on referrals in a timely manner.
• Evaluate the consumer’s/collaterals’ satisfaction with the service(s) via satisfaction surveys and follow up with any issues and concerns in a timely matter. Document follow up that was required to maintain satisfaction of the service delivery process.
• Maintain electronic clinical record as it pertains to Article 16 services including documentation of communication with MSC’s and other approved contacts.
• Ensure that through the treatment plan review and semiannual review that continued need for service is provided and that services provided through the People Inc. Article 16 main site and its satellite sites are integrate with no duplication of service provided elsewhere.                            • Review the clinical record and provide explanations for its content (conduct record reviews using the file review form).
• Ensure that the annual physician reviews, and the annual review of services that the treatment coordinator completes continue to document the need for the service(s) provided.
• Conduct semi-annual and annual reviews to integrate all services that the consumer is receiving through the People Inc. Article 16 clinic.
• Participate in Quality Assurance Activities.

• Promotes positive image of the Elmwood Health Center and People Inc.
• Initiates and problems solves utilizing available resources within level of responsibility.
• Contributes to a positive work environment by interacting in a courteous, pleasant, and professional manner. Functions as a member of an interdisciplinary team through utilization of communication skills and cooperation.
• Demonstrates flexibility by understanding different views. Adapts approach as situations change and accepts changes in the organization and job description requirements
• Supports People Inc Art 16 and People Inc. by accepting additional tasks outside of job description such as special projects, public education, etc.
• Plans time off in advance and manages PTO according to department guidelines.

MINIMUM QUALIFICATIONS:

• Bachelor’s Degree in Nursing, Social Work or Education required, Master’s preferred.
• 3-5 years of experience working with the Developmentally Disabled.

SUPERVISORY RESPONSIBILITIES:

• While this position has no direct supervisory responsibility, the treatment coordinator will provide guidance to a multidisciplinary team responsible to provide services to the Developmentally Disabled.
