Apply today for our TVAR Coordinator opportunity. **Job Summary and Qualifications** The Coordinator of the TAVR Program utilizes discretion and independent judgment to facilitate seamless care from referral to follow-up for patients who are candidates for TAVR therapy; applies nursing process to the provision of care for this subset of patients; manages and coordinates all aspects of the TAVR program; facilitates a team approach and collaborates with the multidisciplinary TAVR team to establish a plan of care starting from initial interventional evaluation and following the continuum of care including pre[1]procedure, post-procedure, and continued follow-up post TAVR implant; works with the patient and family to provide teaching, education and support throughout the TAVR process. Assist in leading the facility to accreditation compliance and regulatory alignment, improvement on national and state/local public-reported measures, focused clinical reporting, and compliance with HCA supported national programs. Ensures maintenance of established electronic database, and submits patient demographic, medical and progress report information within designated time frames as required by local, state, and federal agencies. Participates in the department quality improvement program and strives to optimize patient outcomes through patient education and clinical expertise. Specific responsibilities will include scheduling outpatient evaluations and follow-up, implantation scheduling, maintaining valve database, and overseeing documentation creation of order set, etc.) of the patient experience. The successful candidates will work with a dedicated team of physicians, nurses, technicians, staff and administrators, ensuring optimal care and efficiency for patient with valve disease. If that is you, come be a part of what makes us great and apply today! + You will Coordinate the referral process and introduction of patients into the Heart Valve Center for purposes of evaluation and assessment + You will guide and direct the continuum of care for valve patients from admission through discharge, including post-discharge care and follow-up as appropriate for this patient population. + You will actively participate in the TVT registry to assure that data related to valve patients is reported accurately and appropriately for purposes of Quality Improvement. + You will develop and implement the educational materials and support needed by the patient and family and assures that patients and families receive appropriate educational counseling to assist them in physical and psychosocial progress towards outcomes + You will participate in Performance Improvement/research activities designed to improve patient care, and/or organizational processes + You will provide patient access support to clinical staff with respect to scheduling, preparing for meetings and conferences **What qualifications you will need:** + Current RN license in the State of California. + BCLS Certification + ACLS Certification + Certification in Critical Care with Cardiac Surgery Subspecialty or other related certification preferred + Bachelor’s Degree in Nursing with graduation from an accredited school of nursing required + Master’s degree preferred + Minimum of 5 years’ experience with cardiac /vascular surgery and/or invasive cardiology. Skills in physical assessment, lab data interpretation, technological skills and nursing theory and practice are required. + Teaching experience in patient/family and staff education required.
